摘 要:目的探讨全血超敏C-反应蛋白在儿童呼吸道感染中的应用价值。方法选择2015年1月-2015年6月宝鸡市中心医院收治的患儿230例,按照病因分为细菌性肺炎组74例,病毒性肺炎组62例,化脓性扁桃体炎组51例,哮喘组43例。同时选择健康体检儿童100例作为对照组。采集静脉血,使用ASTEP分析仪检测全血超敏C-反应蛋白水平,2组比较采用t检验,多组间比较采用方差分析。结果病毒性肺炎组、哮喘组的hs-CRP结果与对照组比较差异无统计学意义(P>0.05)。细菌性肺炎组、化脓性扁桃体炎组的hs-CRP结果与对照组比较明显增高,差异有统计学意义(P<0.01)。4种呼吸道疾病全血hs-CRP差异有统计学意义(P<0.05)。细菌性肺炎组和化脓性扁桃体炎组在治疗后hs-CRP较治疗前显著下降,差异有统计学意义(P<0.01)。而病毒性肺炎组和哮喘组hs-CRP比较差异无统计学意义(P>0.05)。结论超敏C-反应蛋白对儿童呼吸道感染的诊断及鉴别诊断具有重要临床意义,对儿童呼吸道感染的治疗方案选择和疗效评估有一定的临床应用价值。Objective To investigate the application value of whole blood C - reactive protein in children with respiratory tract infections. Methods From January 2015 to June 2015, 230 children patients in Baoji Central Hospital were chosen and grouped as bacterial pneumonia group with 74 cases, viral pneumonia group with 62 cases, suppurative tonsillitis group with 51 cases, and asthma group with 43 cases, according to etiology. At the same time, 100 children for health examination were se- lected as the control group. Collecting venous blood, and using the ASTEP analyzer to detect the serum C - reactive protein lev- el, the two groups were compared with t test, and the analysis of variance was used in the comparison between the two groups. Results The differences among bacterial pneumonia group, asthma group and the control group were not statistically significant (P 〉0.05 ). Bacterial pneumonia group and suppurative tonsillitis group significantly increased, compared with the control group, and the differences were statistically significant (P 〈 0.01 ). There was statistical significance on the differences in whole blood hs - CRP levels of the 4 kinds of respiratory diseases ( P 〈 0.05 ). The hs - CRP levels in bacterial pneumonia group and suppurative tonsillitis group decreased more significant after treatment than before treatment, and the differences were statistically significant ( P 〈 0.01 ). And there was little change of hs - CRP between viral pneumonia group and asthma group, and the difference was not statistically significant ( P 〉 0.05 ). Conclusion High sensitivity C - reactive protein in children with respiratory tract infection has important clinical significance in the diagnosis and identification, and have certain clinical ap- plication value in treatment and curative effect evaluation in children with respiratory tract infection.
