2008—2013年湖北省新发现HIV/AIDS病人首次CD_4检测现状分析

摘　　要：目的了解2008—2013年湖北省新发现HIV/AIDS病人首次CD4检测情况和及时性。方法使用2009—2014年每年1月下载的湖北省艾滋病网络直报病例报告卡和2009—2014年每年7月下载的湖北省艾滋病随访管理数据库,对相关数据进行统计分析。结果 2008—2013年湖北省新发现HIV/AIDS病人首次CD4检测比例为78.0%(6 329/8 113),CD_4平均值为(303.0±286.0)个/μL,检测及时率(14天内检测)为59.3%(3 755/6 329);检测比例呈逐年上升趋势(P<0.001)。现住址地区类别为本县区和本市其他县区的病例CD_4检测比例和检测及时率均较高;通过专题调查、医疗机构和其他方式发现的病例CD_4检测比例和检测及时率均较低;大专以下文化程度、来源于受血(制品)前检测和无偿献血人员体检者CD_4检测比例较低;传播途径为注射毒品、来源于戒毒/劳教/羁押人员体检和无偿献血人员检测者CD_4检测及时率较低。结论近年来湖北省新发现HIV/AIDS病人CD4检测比例和及时率有所上升。应该适当增加CD_4检测设备、合理布局检测点和加强人员培训,进一步提高病人的CD_4检测比例和及时率。Objective To analyze the first CD4 count test and its timeliness of newly detected HIV/ AIDS cases from 2008 to 2013 in Hubei Province. Methods Data from HIV/AIDS Case Reporting Cards and Follow-up Cards were obtained from China Information System for HIV/AIDS control and prevention in January and July from 2009 to 2014, and were statistically analyzed. Results Among the newly detected HIV/AIDS from 2008 to 2013, 78.0% (6329/8113) received CD4 count test. Of those receiving CD4 count test, 59.3% (3755/6329) were tested within 14 days. The mean value of CD4 counts was 303.0±286.0cell/μl. The proportion of CD4 count test increased yearly from 2008 to 2013.Both the proportion and timeliness rate of CD4 count test among cases resided in the city and the other districts of the city were higher, but both of them were lower among cases diagnosed in special survey, medical institution and others. The patients who had education level less than college or diagnosed in blood screening had lower proportion of CD4 count test. The cases infected by injecting drug use, diagnosed in drug detoxification centers/detention/custody institutions or in screening in blood donor had the low timeliness rate. Conclusion The proportion and timeliness rate of CD4 count test increased yearly in recent years. However, for further improvement we should increase and distribute the CD4 count test facilities appropriately and train the laboratory personnel.
