特异性免疫治疗对哮喘儿童生命质量及肺功能的影响

摘　　要：目的:探讨特异性免疫治疗对哮喘儿童生命质量及肺功能的影响。方法:将我院哮喘中心建档管理的哮喘儿童根据治疗情况分为两组,研究组(26例)在常规《全球哮喘防治创议》(GINA)治疗方案的基础上进行特异性免疫治疗(SIT);对照组(16例)按照常规GINA方案治疗。入组及治疗满1年时由家长填写儿童生命质量量表体系(Peds QLTM)3.0哮喘模块中文版,比较两组入组时和治疗满1年后生命质量得分及肺功能检测的结果。结果:治疗1年后,研究组Peds QL症状分、治疗分、沟通分及总分均有所增加(P<0.05),忧虑分虽增加,但差异无统计学意义(P>0.05)。对照组Peds QL各维度和总分变化差异均无统计学意义(P>0.05)。两组入组时和治疗1年后的肺功能指标比较,差异均无统计学意义。结论:SIT比传统的激素吸入治疗更能提高患儿的生命质量(QOL)症状分、治疗分、沟通分和总分,两种治疗方法对QOL忧虑分和肺功能指标影响的差异尚未发现有统计学意义。应综合考虑包括患儿生命质量在内的实际情况选择治疗方式。Objective： To explore the effect of allergen specific immunotherapy on the quality of life and pulmonary function in children with asthma. Methods： According to the treatment asthma children were divided into two groups. The study group（ 26 cases） was treated on the basis of conventional Global Initiative for Asthma（ GINA） and specific immune therapy（ SIT）,and control group（ 16 cases） was only accepted conventional GINA treatment. Their parents filled out the asthma children＇ s quality of life scale Peds QLTM3. 0 module（ Chinese version）,the scores and the lung function were compared between two group when enrolled and after 1 year＇ s treatment. Results： After 1 year＇ s treatment for the study group,the Peds QL symptoms points,treatment points,communication points and the total scores increased（ P〈0. 05）. The Peds QL anxiety points increased no significantly（ P〈0. 05）. For the control group,the Peds QL each dimension and the total score changes were not significantly（ P〈0. 05）. For the two groups the differences of FVC,FEV1,PEF,MEF,PD20FEV1 when enrolled and after 1year＇ s treatment had no significance. Conclusion： SIT can improve more the QOL of symptoms,treatment,communication and total score,than that of traditional hormone inhalation therapy. The impact on QOL anxiety points and the lung function index has not been found to be significant. The proper treatment should be selected by the exact situation of the children with asthma,such as,the quality of life.
