质粒介导鼠伤寒沙门菌耐喹诺酮类药物的研究

摘  要:目的了解北京市密云县鼠伤寒沙门菌(Salmonella typhimurium,STM)的耐药性及质粒介导的STM耐喹诺酮类药物机制,为STM的防控提供科学依据。方法 2010年1月-2014年12月,收集腹泻患者的STM共42株。采用微量肉汤稀释法对9种抗菌药物进行体外药敏试验,采用PCR扩增质粒介导的喹诺酮耐药基因即qnr和acc(6')-Ib-cr,接合试验验证qnr基因的转移性。结果 42株STM喹诺酮耐药率高于头孢类抗菌药物,26株STM为喹诺酮类和头孢类多重耐药。其中,10株STM携带质粒介导基因,qnr基因经接合后,受体菌获得大部分的耐药性,但其质粒编码的耐药表型与耐药质粒并不呈现严格的对应关系。结论喹诺酮类药物不适合于STM的感染治疗,同时应对头孢类抗菌药物敏感性下降的菌株进行监测。Objective To analyze the drug resistance mechanism of plasmid-induced Salmonella typhimurium( STM) to quinolones,so as to provide scientific basis for the prevention and control of STM in Miyun,Beijing. Methods A total of 42 STM strains were collected from diarrhea patients from January 2010 to December 2014. In vitro drug sensitivity test was conducted on9 antibiotics by broth dilution method. Plasmid-induced gene resistant to quinolones,that is qnr and acc( 6')-Ib-cr,were amplyfied by PCR method. Conjugation experiment was carried out to test the transfer. Results The resistance rate of 42 STM strains to quinolone was higher than that of cephalosporins,among which 26 trains of STM were multi-drug resistance of quinolone and cephalosporins. 10 STM strains were carrying PMQR gene. By conjugating qnr gene,recipient strains got most of the drug resistance,but the drug resistance phenotype of plasimid encoding were not related to the drug resistant plasmid. Conclusion Quinolones are not suitable for the treatment of STM infection. The strains with resistance to ceftraxone and reduced-sensitivity should be monitored.
