实时三维超声心动图对法洛四联症患者右心室收缩功能变化的评估价值研究

摘　　要：目的：分析实时三维超声心动图对法洛氏四联症（ToF）患者右心室收缩功能变化的评估价值。方法：收集在本院接受手术治疗的TOF患儿63例作为观察组对象，另取同期在本院进行健康检查的正常儿童72例作为正常对照组，均进行二维及三维超声检查，对比观察TOF患儿的右心室收缩功能情况。结栗：TOF患者的术前左肺动脉内径5．38～9．65mm，右肺动脉内径5．34～9．15mm，肺动脉指数（Nakata指数）平均（160．92±21．58）mm2／m2；术前TOF组右心室舒张末期容积（RVEDV）、右心室收缩末期容积（RVESV）值大于正常对照组，右心室射血分数（RVEF）值小于正常对照组，差异具有统计学意义（P〈0．05）；观察组的术后6月RVEDV、RVESV增加，RVEF下降，与术前相比，差异具有统计学意义（P〈0．05）；TOF患者术前三维超声RVESV、RVEDV值高于二维超声检查结果，差异具有统计学意义（P〈0．05）；TOF患者术前RVEF与Nakata指数存在明显的正相关关系（P〈0．05）。结论：实时三维超声心动图可以准确客观的评估法洛四联症患者的右心室收缩功能变化，有助于治疗效果及远期预后的评估。Objective： To analyze the assessment value of real-time three-dimensional echocardiography on right ventric- ular systolic function in patients with Tetralogy of Fallot. Methods： A total of 63 children with Tetralogy of Fallot undergoing surgical treatment in our hospital were chosen as observation group, 72 healthy children receiving health examination in our hospital during the same period were chosen as normal control group, all of them received two-dimensional and three-dimen- sional ultrasound examination, and right ventricular systolic function of TOF children was compared and observed. Results= TOF patients＂ preoperative left pulmonary artery inner diameter was 5.38- 9.65 mm, right pulmonary artery inner diameter was 5.34-9.15mm, average pulmonary artery index （Nakata index） was （160.92 ± 21.58） mm2/m2; TOF patients＇ preop- erative RVEDV and RVESV values were greater than those of normal control group and RVEF value was less than that of nor real control group （P〈0.05）; 6 months after undergoing surgery, RVEDV and RVESV increased, while RVEF decreased showing significant difference comparing with that before surgery （P〈0.05） ; TOF patients＂ preoperative three-dimensional ul- trasound RVESV and RVEDV values were higher than results of two-dimensional ultrasound examination （P〈0.05）; TOF patients preoperative RVEF had significant positive correlation with Nakata index. Conclusion： Real-time three-dimensional echocardiography can accurately and objectively assess right ventricular systolic function in patients with Tetralogy of Fallot, and contributes to the assessment of treatment effect and long-term prognosis.
