摘 要:目的:探讨血β_2微球蛋白(β_2-MG)与窒息新生儿早期肾损害相关性。方法:选取本院收治的窒息足月新生儿84例,根据Apgar评分标准,分为重度窒息组(n=36)和轻度窒息组(n=48),同时选取正常足月新生儿40例,观察各组血尿β_2-MG、白蛋白(Alb)、血尿素氮(BUN)、肌酐(Cr)等水平。结果:重度窒息组β_2-MG最高,其次为轻度窒息组,对照组最低,差异具有统计学意义(P<0.05);重度窒息组Alb、BUN和Cr明显高于轻度窒息组和对照组,差异具有统计学意义(P<0.05);轻度窒息组和对照组Alb、BUN和Cr比较,差异无统计学意义(P>0.05);三组出生48h血β_2-MG、Alb、BUN和Cr比较,差异均有统计学意义(P<0.05),其中重度窒息组β_2-MG、Alb、BUN和Cr为最高,对照组最低;重度窒息组β_2-MG最高,为(7.12±1.32)mg/L;血β_2-MG与尿β_2-MG呈正相关(r=0.411,P<0.05)。结论:血β_2微球蛋白在判断窒息新生儿早期肾损害较灵敏,具有临床实用价值。Objective: To investigate correlation between serum 132 microglobulin and early kidney injury post neonatal asphyxia. Methods= A total of 84 suffocation full-term newborns treated in our hospital were divided into severe asphyxia group (n=36) and mild asphyxia group (n= 48) according to Apgar standards. Meanwhile 40 healthy full-term newborns were en- rolled as control group. Blood and urinary β2-MG, albumin (Alb), blood urea nitrogen (BUN) , creatinine (Cr) level were ob served in both groups. Results: In the severe asphyxia group, β2-MG was the highest, followed by mild asphyxia group, and it was the lowest in the control group (P〈0.05). Alb, BUN and Cr levels of the severe asphyxia group was significant higher than that of the mild asphyxia group and that of the control group (P〈0.05), but no significant difference in Alb, BUN and Cr levels was found between the mild asphyxia group and the control group (P〉0.05). Significant difference in 48 h β2-MG, Alb, serum BUN and Cr levels was found among the three groups, of which the highest levels were found in severe asphyxia group, and the lowest levels were in the control group. In the severe asphyxia group, serum β2-MG level was positively corre- lated with urine β2-MG level(r=0. 411 ,P〈0.05). Conclusion= Serum β2-2 mieroglobulin is a sensitive indicator for diagnosis of early kidney injury post neonatal asphyxia.
