冠状动脉侧枝循环的建立与血清脂联素水平关系研究

摘　　要：目的：比较冠状动脉侧枝循环建立情况不同的冠心病患者血清脂联素水平,探讨促进冠状动脉侧枝循环建立的因素。方法：将87例接受冠脉造影患者（男43例,女34例）按其造影结果以rentrop分级方法分为：阴性组（A组）,3支主要冠脉（右冠、前降支、回旋支）至少有1支狭窄≥95%,侧枝循环为0-1级者;阳性组（B组）,3支主要冠脉至少有1支狭窄≥95%,侧枝循环为2-3级者;对照组（C组）,冠脉造影未见冠脉狭窄者。通过ELISA法测定患者血清脂联素水平。结果：血清脂联素水平B组高于A组（P=0.001）,C组高于A组（P=0.001）。结论：冠状动脉侧枝循环的建立与血清脂联素水平具有线性关系,侧枝循环建立较好者脂联素水平更高。脂联素可能是冠状动脉侧枝循环建立的一个独立预测因子及影响因子,有助于对冠心病患者预后的评估。Objective： To explore ways of promoting the factors of coronary collateral circulation,analyzing the relationship of the coronary collateral circulation establishment and level of serum adiponectin. Methods： Patients undergoing coronary angiography were divided into the following three groups according to the angiographic results rentrop classification system： the negative group（ group A）,stenosis of at least one of the three main coronary artery（ right coronary,left anterior descending artery,circumflex artery） was more than 95% and the collateral circulation be classified for 0-1 grade; the positive group（ group B）,stenosis of at least one of the three major coronary was more than 95% and collateral circulation be classified for 2-3 grade; the control group（ group C）,there was no coronary stenosis under coronary angiography. Eventually 87 cases were selected including 43 males and 34 females. The level of serum adiponectin was measured by ELISA. Results： Adiponectin levels of group B,C were significantly higher than that of group A（ P = 0. 001）. Conclusions： The establishment of the coronary collateral circulation,and serum adiponectin levels are relevant,adiponectin may be an independent predictor of coronary collateral circulation and impact factor. Contribute to the assessment of the prognosis of patients with coronary artery disease.
