健康成人新鲜血清补体C3、C4参考范围的建立

摘　　要：目的：探讨保存时间、性别、年龄对补体C3、C4水平的影响,并建立深圳地区健康成人新鲜血清补体C3、C4参考范围。方法：收集1 058例健康体检结果正常成人血清,依照年龄段分为3组：青年组（18-40岁）351例,中年组（41-60岁）375例,老年组（〉60岁）332例,于8 h内进行测定;其中40份血清标本分成两份,分别于8 h内和24 h检测,应用SPSS对检验结果进行统计分析。结果：血清保存于4℃24 h,补体C3、C4含量明显升高（P〈0.01）;新鲜血清C3含量在女性组中明显升高,与男性比较差异具有统计学意义（P〈0.05）,而新鲜血清C4含量在女性、男性之间无明显差异（P〉0.05）;新鲜血清C3、C4含量与年龄无关（P〉0.05）;新鲜血清C3男性参考范围为0.777-1.461 g·L^-1,女性参考范围为0.744-1.315 g·L^-1,新鲜血清C4整体人群参考范围为0.095-0.351 g·L^-1。结论：血清C3含量在不同保存时间及性别间存在显著差异,而C4仅在不同保存时间存在明显差异,因此应建立适合本地区实际临床情况的参考范围,以有助于提高医疗质量。Objective： To investigate whether storage time,gender and age influence the measurement of C3,C4,and establish the fresh serum C3,C4 reference interval for healthy adults in Shenzhen. Methods： According to the age of people,serum samples of 1 058 healthy examination crowed were divided into the the youth group（ 18-40 years old,351 cases）,middle-aged group（ 41- 60 years old,375 cases） and older-aged group（ 〉60 years old,332 cases）. Serum C3,C4 levels in the three groups were determined in 8 hours. Serum samples for 40 cases were divided into two parts,and serum C3,C4 levels were determined in 8 hours and 24 hours. Tested results were analyzed. Results： Compared with the fresh serum for 8 hours,levels of C3,C4 increased obviously in the serum stored at 4 ℃ for 24 hours（ P 〈0. 01）. Fresh serum C3 level increased obviously for female people,and significant difference was found compared with male people（ P〈0. 05）. No significant difference of C4 level was found between male and female people（ P〉0. 05）. C3,C4 levels had nothing to do with age（ P〉0. 05）. Fresh serum C3 reference interval was 0. 777- 1. 461 g · L^- 1for male people,and 0. 744- 1. 315 g · L^- 1for female people. Fresh serum C4 reference interval was 0. 095- 0. 351 g · L^- 1. Conclusion： Serum C3 level is significantly different in different storage time and gender. But serum C4 level is significantly different in different storage time. Therefore a suitable reference interval is established for local clinical situation,which help to improve the quality of medical treatment.
