摘 要:目的对比生物型与骨水泥型全髋关节置换术治疗老年骨质疏松的临床效果。方法选取2013年1月—2015年1月收治的全髋关节置换术老年骨质疏松髋部骨折患者200例作为研究对象,根据骨折情况分为A组120例与B组80例。A组给予骨水泥型髋关节置换术治疗,B组给予生物型髋关节置换术治疗,对比两组髋关节功能恢复情况、术后疼痛情况及术后并发症发生情况。计数资料组间比较采用t检验,组内比较采用配对t检验,计量资料采用χ2检验,P<0.05为差异有统计学意义。结果 A组和B组术后90 d随访时髋关节功能评分分别为(90.5±8.6)、(83.7±8.5)分,均优于治疗前的(73.5±6.8)、(74.0±6.9),差异均有统计学意义(均P<0.05)。且两组术后髋关节功能评分比较差异有统计学意义(P<0.05)。A组术后优良率为88.3%,明显优于B组的71.3%,差异有统计学意义(P<0.05)。A组发症发生率为25.8%,显著高于B组的3.8%,差异有统计学意义(P<0.05)。结论对于老年骨质疏松症患者行全髋关节置换术时采用骨水泥型术后疗效明显优于生物型,但有诱发静脉血栓和骨水泥危象的风险。Objective To compare the clinical effects of biotype and acrylic cement type total hip arthroplasty to elderly patients of osteoporosis. Methods 200 cases of senile osteoporotic hip fracture patients from January 2013 to January 2015 were selected and randomly divided into group A(n=120) and group B(n=80).The patients in group A were treated with cemented hip replacement, and the patients in group B were treated with biological hip replacement. The functional recovery, postoperative pain and postoperative complications were compared between the two groups. Measurement data between groups were compared using t test, in group were processed by paired t test, count data using chi square test, P〈0.05 was considered statistically significant.Results The function scores of hip joint were(90.5±8.6) and(83.7±8.5)points in group A and group B, superior to that before treatment[(73.5±6.8),(74.0±6.9)points], the differences were statistically significant(all P〈0.05). The functional score of hip joint after treatment was significantly different in the two groups(P0.05). The excellent and good rate of group A was 88.3%, which was better than that in group B(63.3%),the difference was statistically significant(P〈0.05).The complication rate of group A was 25.8%,higher than that in group B(3.8%),the difference was statistically significant(P〈0.05).Conclusion As for total hip arthroplasty to elderly patients of osteoporosis, postoperative efficacy of acrylic cement type is better than that of biotype, but it has the risk of inducing venous thrombosis and acrylic cement crisis.
