机械通气患者应用不同气道湿化方法临床效果研究

摘　　要：【目的】探讨机械通气患者应用不同主动加温湿化装置的气道湿化效果。【方法】2013年1月至2014年5月,选择我院呼吸与重症医学科建立人工气道需要机械通气治疗的危重患者121例,随机分为两组,A组(n=61)采用MR810湿化装置,B组(n=60)采用MR850湿化装置。分别测量各组患者48 h后呼吸频率、Pa O2、s PO2、心率;采用气道分泌物评分评价痰液黏稠度;监测两组患者肺部感染控制的时间。【结果】两组患者在性别、年龄、病种、急性生理与慢性健康评分系统Ⅱ评分、诊断等方面差异均无统计学意义(P>0.05)。两组患者的呼吸频率、Pa O2、s PO2、心率、痰液黏稠度、湿化效果及肺部感染控制时间比较差异均有统计学意义(P<0.05)。【结论】与MR810湿化装置比较,MR850湿化装置更适用于机械通气患者,能为患者提供更符合生理需求、更理想的气道湿化方法。【Objective】Discusses the application of mechanical ventilation in patients with different active plus temperature humidity device of airway of wetting effect.【Methods】Affiliated hospital of armed police logistics university respiratory and intensive medicine,between January 2013 and May 2014 set up artificial airway need mechanical ventilation treatment of 121 cases of critically ill patients,divide into 2 groups randomly divided into group A（n=61） with MR810 of wetting device, group B（n=60） using MR850 of wetting device.Groups of patients were measured after 48 h breathing rate, Pa O2, s PO2, heart rate; The airway secretions marking（AWSS） evaluation of sputum viscosity; Monitoring of two groups of patients with pulmonary infection control time. 【Results】Health evaluation system（APACHE Ⅱ） score, the respect such as diagnosis differences had no statistical significance（P〉0.05）. Two groups of patients with respiratory frequency, Pa O2, s PO2, heart rate, sputum viscosity, wetting effect and lung infection control time comparative differences are statistically significant（P〈0.05）.【Conclusions】Compared with MR810 humidity device, MR850 of wetting device is more suitable for patients with mechanical ventilation, provide patients with more accord with the physiological requirements and more ideal method of airway moist.
