老年冠心病患者术前HO-1水平预测非心脏手术围术期心血管事件的价值

摘　　要：目的初步探讨老年冠心病患者HO-1水平与非心脏手术围手术期心血管事件的相关性。方法收集2009年7月~2014年5月于我院择期进行中高危非心脏手术的446例老年冠心病患者病历资料,术前1周内抽取肘静脉血样留样保存。利用RCRI对患者围术期心血管事件危险性进行评价,ELISA检测患者血清HO-1水平。统计分析术中、术后心血管事件病例与非心血管事件病例临床特征、RCRI指数及c Tn I水平与术前HO-1的相关性。结果糖尿病、高血压合并症及高龄是围术期心血管事件的危险因素;RCRI指数及HO-1水平围术期心血管合并症发病率呈正相关,术后血清c Tn I水平与术前血清HO-1水平呈正相关。结论 HO-1水平可以用作老年冠心病患者非心脏手术围术期心血管事件的预测因子,其敏感性和特异性的确定还有赖于进一步研究。Objective To explore the correlation between HO-1 levels and perioperative cardiovascular event of non-cardiac surgery in aged patients undergoing coronary heart disease.Methods A total of 446 aged patients with coronary artery disease admitted to our hospital for major elective non-cardiac surgery were enrolled from July 2009 to May 2014.The medical records were collected,and the preoperative venous blood samples were taken within one week before the operation and preserved for detection.Revised cardiac risk index was performed to assess the risk of perioperative cardiovascular events in the patients.ELISA assays were used to detect serum HO-1levels of those cases.Clinical features,RCRI,c Tn I and HO-1 levels were statistically analyzed in cases with intraoperative and postoperative cardiovascular events and cases without cardiovascular events.Results Diabetes,high blood pressure and age were risk factors for perioperative cardiovascular events; RCRI index and the HO-1 levels were positively correlated with the perioperative cardiovascular complications.Conclusion HO-1 levels can be functioned as a predictor of perioperative cardiovascular events during non-cardiac surgery of patients undergoing coronary heart disease,but the sensitivity and specificity of HO-1 as a predictor need to be confirmed by further research.
