血清LH峰值、IGF-1和IGF-BP3水平在特发性中枢性性早熟中的诊断价值

摘　　要：目的探讨血清促黄体生成素(LH)峰值、胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白3(IGF-BP3)水平在特发性中枢性性早熟(ICCP)患者中的诊断价值。方法选择75例性早熟女童为研究对象,匹配75例正常青春期发育女童为对照组。前者再分为特发性中枢性性早熟组(n=30)和单纯乳房发育组(n=45)。比较三组血清IGF-1、IGF-BP3和LH峰值的水平。结果特发性中枢性性早熟组血清LH峰值水平显著高于单纯乳房发育组(P<0.05),IGF-1和IGF-BP3水平显著高于单纯乳房发育组和对照组(P<0.05)。而单纯乳房发育组与对照组IGF-BP3水平无统计学差异(P>0.05)。结论血清IGF-1、LH峰值可用于ICCP患者的鉴别诊断。Objective To investigate the diagnostic value of the peak of serum luteinizing hormone（ LH）,and the changes of insulin-like growth factor-1（ IGF-1） and insulin- like growth factor binding protein- 3（ IGF-BP3） levels in idiopathic central precocious puberty（ ICPP） patients. Methods 75 cases of precocious puberty girls were collected for the study. 75 cases of matched normal puberty girls were collected in the control group. The former group was subdivided into ICPP group（ n = 30） and breast development alone group（ n = 45）. Comparison of the three groups in serum IGF- 1,IGF-BP3 levels and LH peak was undertaken. Results LH peak in ICPP group was significantly higher than breast development group（ P 〈0.05）. In ICPP group,serum IGF- 1 and IGF- BP3 levels were significantly higher than breast development alone group and the control group（ P 〈 0.05）. There was no statistical difference in serum IGF- BP3 level between simple breast development group and control group（ P 〉 0.05）. Conclusion Serum IGF-1 level and LH peak can be used for differential diagnosis of ICPP patients,which is worthy of clinical use.
