热毒宁注射剂联合利巴韦林气雾剂治疗手足口病疗效观察

摘　　要：目的探讨用热毒宁注射剂联合利巴韦林气雾剂治疗手足口病的临床疗效。方法选择2012年5月至2014年6月就诊的96例手足口病患儿为研究对象,将其随机分为对照组和治疗组患儿,各47例。对照组患儿给予利巴韦林气雾剂治疗,治疗组患儿则给予热毒宁注射剂静脉滴注联合利巴韦林气雾剂治疗,1次/d,连续治疗5 d。治疗后观察两组患儿口腔疱疹、发热、厌食、哭闹、手足臀部疱疹、咳嗽等临床症状的改善情况,对患儿的临床征候给予评分,比较征候的变化情况;检测患儿超敏C反应蛋白(hs-CRP)、白细胞计数(WBC)及肝功能相关指标丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)水平变化情况;统计患儿发热消退的时间、皮疹消失的时间、体温恢复至正常的时间、治疗时间。结果两组患儿经治疗后口腔溃疡、发热、厌食、哭闹、皮疹、咳嗽等临床症状较治疗前有所缓解,征候积分较治疗前有所降低,以治疗组的变化程度更为显著,差异有统计学意义(P<0.05);治疗后两组患儿hs-CRP、WBC、ALT及AST水平均较治疗前有所降低,治疗组患儿在降低上述指标方面明显优于对照组患儿,差异有统计学意义(P<0.05);治疗组患儿的发热消退的时间、皮疹消失的时间、体温恢复至正常的时间及治疗时间明显短于对照组患儿,差异有统计学意义(P<0.05)。结论热毒宁注射剂联合利巴韦林气雾剂能有效缓解手足口病患儿的临床症状,能有效改善患儿的肝功能,缩短病程,抗炎作用显著,疗效确切,值得临床深入研究。Objective To explore the clinical efficacy of application of Reduning injection combined with ribavirin aerosol in treatment of patients with hand- foot- mouth disease. Methods A total of 96 patients with hand- foot- mouth disease during May 2012 to June 2014 were randomly divided into control group（ n = 48 cases） and trial group（ n = 48 cases）. Patients in control group were treated with ribavirin aerosol;while patients in trial group were treated with Reduning injection,1 time / day,5 days as a treatment course. The improvement in clinical symptoms including mainly ulcer in oral cavity,fever,anorexia,cry,rashes,coughing and so on had been observed and compared between these 2 groups,the improvement in main symptoms was evaluated by scoring. The changes in levels of CRP,WBC,ALT and AST were compared between these2 groups. The duration for remission of fever and rashes,the duration for temperature turn to normal and the duration of treatment were observed and compared between these 2 groups. Results The main clinical symptoms of patients in these two groups were significantly improved compared with those before treatment,and the symptom scores were significantly decreased,the degree of changes in patients of trial group was more prominent,and the difference was significant（ P 〈 0. 05）. The decrease in levels of CRP,WBC,ALT and AST was very clear,the degree of changes in patients of trial group was more pronounced,and the difference was significant（ P 〈 0. 05）. The duration for disappearance of fever and rashes,the time for temperature turned back to normal and the course of treatment in patients of trial group were shorter than those of patients in control group,and the difference was significant（ P 〈 0. 05）. Conclusion The treatment of patients with hand- foot- mouth disease by application of Reduning injection and ribavirin aerosol can significantly relieve their clinical symptoms,improve the function of liver and shorten the duration of disease course.
