动脉瘤体穿刺减压在巨大颅内动脉瘤夹闭术中的应用价值

摘　　要：目的探讨动脉瘤体穿刺减压在巨大颅内动脉瘤夹闭术中的应用价值。方法选择14例颅内巨大动脉瘤的患者,术中采用动脉瘤体穿刺减压,术后采用Glasgow预后评分评价疗效。结果 14例患者术中临时阻断血管的时间范围8~16 min,平均12.45±2.31 min。术后半年随访发现,患者预后良好12例（85.71%）,差1例（7.14%）,死亡1例（7.14%）。结论动脉瘤体直接穿刺减压有利于巨大颅内动脉瘤手术夹闭,从而改善患者治疗效果,值得临床应用。Objective To investigate the value of arterial puncture decompression in giant intracranial aneurysm clipping surgery. Methods 14 patients with giant intracranial aneurysm were enrolled. Arterial puncture decompression was used in intraoperative giant intracranial aneurysm surgery. The Glasgow outcome score（ GOS） was used to evaluate the therapeutic efficacy. Results In 14 patients,temporary occlusion time of the parent artery was 8 ~ 16 min,with average of 12. 45 ± 2. 31 min. After six- month follow- up,12 patients had relatively good outcome（ 85. 71%）,one had poor outcome（ 7. 14%） and one patient died（ 7. 14%）. Conclusion Puncture decompression during giant intracranial aneurysm clipping surgery has a better therapeutic efficacy,which is worthy of clinical application.
