摘 要:目的 脂联素对肾缺血再灌注小鼠肾纤维化的影响.方法 雄性C57BL/6小鼠48只,8周龄,体重20~25 g,采用随机数字表法分为4组(n=12):对照组(C组)、脂联素组(APN组)、缺血再灌注组(I/R组)和缺血再灌注+脂联素组(I/R+APN组).采用阻断左侧肾蒂30 min再灌注法制备肾脏缺血再灌注损伤模型.I/R+APN组于缺血前15 min和再灌注每24 h腹腔注射APN蛋白球状片段5 μg/g,连续2周;C组分离左侧肾脏,但不夹闭肾蒂,C组和I/R组与相应时点腹腔注射等量生理盐水.APN组分离左侧肾脏,但不夹闭肾蒂,给药方法同I/R+APN组.于再灌注后48 h时,取左侧肾组织,行肾小管损伤评分,采用TUNEL法检测凋亡指数(AI).再灌注2周时取肾组织,分别行天狼星红饱和苦味酸(Sirus red)染色和蛋白定量,计算肾间质胶原沉积面积占视野内肾小管间质总面积的百分比(Sirus red面积百分比),评估左侧肾脏纤维化程度;Western blot法测定Ⅰ型胶原(COL-Ⅰ)、α-平滑肌动蛋白(α-SMA)和纤维连接蛋白(FN)的表达水平.结果 与C组比较,I/R组和APN+I/R组肾小管损伤评分、AI和Sirus red面积百分比升高,肾组织COL-Ⅰ、α-SMA和FN表达上调(P<0.05),APN组上述指标差异无统计学意义(P>0.05);与I/R组比较,APN+I/R组肾小管损伤评分、AI和Sirus red面积百分比降低,肾组织COL-Ⅰ、α-SMA和FN表达下调(P<0.05).结论 脂联素可减轻肾缺血再灌注小鼠肾纤维化.Objective To evaluate the effect of adiponectin on renal fibrosis in mice subjected to renal ischemia-reperfusion (I/R).Methods Forty-eight male C57BL/6 mice, aged 8 weeks, weighing 20-25 g, were randomly divided into 4 groups (n =12 each) using a random number table: control group (group C), adiponectin group (group APN) and group I/R.Renal ischemia was induced by occlusion of the left renal pedicles for 30 min with atraumatic microclips followed by reperfusion.Globular domain of adiponectin 5 μg/g was injected intraperitoneally at 15 min before ischemia and every 24 h during reperfusion for 2 consecutive weeks in group I/R+APN.In group C, the left renal pedicles were only isolated, but not occluded, and the equal volume of normal saline was given at the corresponding time point in groups C and I/R.In group APN, the left renal pedicles were only isolated, but not occluded, and the medication was similar to those previously described in group I/R+APN.At 48 h of reperfusion, 6 mice were sacrificed in each group, the left kidneys were removed for detection of cell apoptosis (using TUNEL) , and the damage to the renal tubules was scored.Apoptosis index (AI) was calculated.The left 6 mice in each group were sacrificed at 2 weeks of reperfusion, and the left kidneys were obtained, the degree of fibrosis was evaluated using Sirus red staining, and the expression of type Ⅰ collagen (Col-Ⅰ), a-smooth muscle actin (α-SMA), and fibronectin (FN) was detected by Western blot.Results Compared with group C, the renal tubular damage score, AI and degree of fibrosis were significantly increased, and the expression of COLⅠ , α-SMA and FN was up-regulated in I/R and APN+I/R groups (P〈0.05), and no significant change was found in the parameters mentioned above in group APN (P〉0.05).Compared with group I/R, the renal tubular damage score, AI and degree of fibrosis were significantly decreased, and the expression of COL-Ⅰ , α-SMA and FN was down-regulated in group AP
