摘  要:目的观察胰高血糖素样肽-1(GLP-1)对原发性高血压大鼠心肌微血管损伤的保护作用。方法120只SHR大鼠随机平均分为4组,分别用于药效实验和细胞实验的对照组和实验组(30只)。药效试验:实验组皮下注射艾塞那肽(GLP-1类似物),每日1次,连续7d,对照组注射生理盐水,第8天测定并比较两组大鼠血压、心率、左心室厚度、微动脉密度、壁腔比值、毛细血管密度和内皮细胞面积的差异。细胞实验:提取实验组和对照组大鼠的心肌微血管内皮细胞(CMECs),消化后接种于6孔板,传2~3代,对照组常规细胞培养,研究组采用GLP-1预处理培养,超氧化物试刹盒检测活性氧(ROS)生成,原位缺口末端标记法(TUNEL)法检测CMECs凋亡。结果药效试验:交验组给药后血压(127.81±10.41)mmHg[1mmHg=0.133kPa,P〈0.05]、心率(363.1±15.9)次/分(P〈0.01)以及微动脉特征指标左心室厚度(10.71±1.20)mm(P〈0.05)、微动脉密度4.48±2.21(P〈0.05)、壁腔比值1.33±0.18(P〈0.05)和内皮细胞面积17.10±5.27(P〈0.01)较对照组均显著降低,但毛细血管密度显著升高159.7±50.1(P〈0.05)。细胞试验:实验组ROS荧光强度0.25,明显低于对照组(P〈0.05),CMECs凋亡率(39.30±3.12)%,明显高于对照组(χ2=10.67,P〈0.05)。结论GLP-1干预能够逆转原发性高血压大鼠左心室肥厚,改善其心脏功能,并降低SHR大鼠壁腔比值,增加毛细血管密度和病变细胞凋亡率,对原发性高血压患者心肌微血管损伤具有一定的保护作用。Objective Explore protective function of glucagon - like peptide - 1 ( GLP - 1 ) in es- sential hypertension patients with microvascular injury. Methods 120 SHR rats were divided into 4 groups, which were study groups and control groups of efficacy trials and cell trials. Efficacy trials:study Group's rats were injected subcutaneously exenatide (GLP-1 analogues) ,control Group's rats were injec- ted with saline, compared the blood pressure, heart rate, left ventrieular thickness, micro-arterial density, wall cavity ratio, capillary density, endothelial cell area. Cell trials:extracted microvascular endothelial cells (CMECs) of rat heart muscle from control group and study group, then CMECs were inoculated in 6-well plates after digested, cultured 2-3 generation, the control group received conventional cell culture, the study group was cultured with GLP-1 pretreatment. Detected the reactive oxygen species (ROS) fluorescence in- tensity and CMECs apoptosis. Results Efficacy trials : study group' s blood pressure ( 127.81 ± lO. 41 ) mmHg [ 1 mmHg = 0. 133 kPa, P 〈 0. 05 ], heart rate ( 363.1± 15.9 )/min ( P 〈 0. 01 ), left ventricular thickness (10.71 ± 1.20) mm (P 〈0.05) ,micro-arterial density 4.48 ±2. 21 (P 〈0.05) ,wall cavity ra- tio 1.33 ± 0. 18 ( P 〈 0. 05 ) and endothelial cell area 17.10 ± 5.27 ( P 〈 0. 01 ) were all lower than control group,but capillary density were higher 159. 7 ± 50. 1 (P 〈 0. 05). Cell test:study group' s ROS fluores- cence intensity 0. 25 were significantly lower than control group (P 〈 0. 05 ). but CMECs apoptosis rate (39. 30 ± 3. 12) % were significantly higher control group (χ2 = 10. 67, P 〈 0. 05 ). Conclusion GLP-1 can reverse left ventricular hypertrophy, improve heart function and reduce wall cavity ratio,increase capillary density and endothelial ceils numbers, which can protect myocardial microvascular injury in essential hypertension patients.
