筋膜扩张器配合输尿管镜钬激光在女性中下段输尿管结石中的临床应用价值(附56例报告)

摘　　要：目的：探讨筋膜扩张器配合输尿管镜钬激光在女性中下段输尿管结石中的临床应用价值。方法：对56例女性中下段输尿管结石患者,采用筋膜扩张器扩张输尿管,通过Peel-away塑料薄膜鞘行输尿管镜钬激光碎石术。结果：56例患者中,一次性碎石成功52例（93%）,4例（7%）患者有碎石块被冲入肾盂,行ESWL治愈,同时以钬激光处理输尿管息肉或肉芽组织28例,手术时间平均30-40min,未出现结石残留或严重并发症。结论：临床应用筋膜扩张器配合输尿管镜钬激光治疗女性中下段输尿管结石能明显缩短手术时间,降低手术风险,提高结石清除率,减少了术后感染、疼痛等并发症,减轻患者痛苦,术后恢复更快。Objective： To investigate the clm;cai appucauuLt u, female middle and lower ureteral calculi. Method： Dilation of ureter was taken by fascia dilator in 56 females who were diagnosed as middle and lower ureteral calculi, then Peel-away plastic fihn sheath ureteroscopic holmium laser lithotripsy followed. Result： Among the 56 cases, disposable gravel success rate was 93; （52/56）. Other four patients （7 % ） were found broken stones rush into the renal pelvis, then ESWL was successfully applied. At the same time holmium laser was used to treat the ureteral polyps or granulation tissue in 28 cases. Average operation time was 30-40 minutes, and no residual stone or severe complication was found. Conclusion： Fascia dilator and ureteroscopic holmium laser in female middle and lower ureteral stones can clinically shorten operation time, reduce the operation risk, improve the stone-free rate, reduce the postoperative complications such as infection, pain, reduce the patients＇ pain and have a faster postoperative recovery.
