丙种球蛋白对儿童川崎病BLyS和APRIL水平的影响

摘　　要：目的 研究丙种球蛋白(IVIG)对川崎病(KD)患儿血浆中B淋巴细胞刺激因子(BLyS)和增殖诱导配体(APRIL)水平的影响。方法 采用ELISA法分别检测IVIG治疗前后的KD患儿(A组,48例)以及非KD患儿(B组,20例)血浆中BLyS和APRIL水平。结果 A组治疗前血浆BLyS和APRIL水平均高于B组[(1761.4±1011.9)ng/L vs.(953.4±158.8)ng/L和(48.1±28.4)ng/ml vs.(22.8±16.8)ng/ml](P<0.05或P<0.01)。与治疗前相比,A组28例获得IVIG治疗前后配对血标本患儿治疗后BLyS水平降低[(1378.9±1005.2)ng/L vs.(1982.3±922.5)ng/L](P<0.05),而APRIL水平升高[(59.8±26.8)ng/ml vs.(38.9±25.2)ng/ml](P<0.01)。结论 IVIG可能通过调节BLyS和APRIL的水平影响KD的病理发展和转归。Objective To investigate the effect of intravenous immunoglobulin （IVIG ） on plasma levels of B‐lymphocyte stimulator （BLyS ） and a proliferation‐inducing ligand （APRIL ） in children with Kawasaki disease（KD） .Methods Plasma levels of BLyS and APRIL were detected by ELISA in 48 children with KD（group A） before and after IVIG treatment and 20 children without KD （group B） .Results Plasma levels of BLyS and APRIL in group A before treatment were higher than those in group B[（1761.4 ± 1011.9） ng/L vs .（953.4 ± 158.8） ng/L and （48.1 ± 28.4） ng/ml vs . （22.8 ± 16.8） ng/ml]（P〈0 .05 or P〈0 .01） .Compared with before ,plasma BLyS was decreased [（1378.9 ± 1005.2） ng/L vs .（1982.3 ± 922.5） ng/L]（P〈0 .05） ,while the level of APRIL was increased after treatment in 28 KD children of group A[（59.8 ± 26.8） ng/ml vs .（38.9 ± 25.2） ng/ml] （P〈0 .01） .Conclusion IVIG may affect the pathological development and disease outcome of KD through regulating plasma levels of BLyS and APRIL .
