摘　　要：目的 探讨麦默通微创旋切系统对体检不可触及乳腺肿块进行微创切除的应用价值.方法 回顾性分析南京鼓楼医院集团宿迁市人民医院68例患者共159枚体检不可触及的乳腺肿块行麦默通微创切除的临床资料,术后6个月复查随访,评价麦默通微创旋切术的应用价值.结果 所有肿块均成功旋切,每枚肿块平均手术时间（14.0±6.3）min,平均旋切（11.0±5.7）次;所有肿块术后均行常规病理检查,其中纤维腺瘤127例,乳腺腺病25例,乳腺腺病伴不典型增生4例,导管内乳头状瘤2例,浸润性导管癌1例;主要手术并发症：局部皮肤瘀斑25例,术后血肿6例,术中出血4例,术后感染1例.良性患者均获临床和超声随访6个月,所有患者均未见肿块残留、复发,瘢痕小而隐蔽.结论 超声引导下麦默通微创旋切技术切除体检不可触及的乳腺肿块,定位准确、操作简单、痛苦小、愈合快、切口隐蔽,值得临床推广应用.Objective To study the application value of Mammotome minimally invasion surgery on non-palpable breast lesion.Methods The clinical data of 68 cases of 159 non-palpable breast lesions underwent ultrasound guided Mammotome minimally invasion surgery were retrospective analyzed.The application value of Mammotome system were evaluated 6 months followed-up after operation.Results All the breast lesions were excised safely and completely by Mammotome system.The average operation time of every lesion was （14.0-± 6.3） minutes and the average number of times of resection was （11.0 ± 5.7） times.Postoperative pathological examination showed all the lesions were benign.There were fibroadenoma in 127 lesions, breast adenosis in 25 lesions, breast adenosis with dysplasia in 4 lesions, intraductal papilloma in 2 lesions and infitrating ductal breast cancer in 1 lesion.The main operation complications after operation were 25 cases of ecchymosis, 6 cases of hematoma, 4 cases of intraoperative hemorrhage, and 1 case of infection.All the patients were followed up for 6 months by physical and ultrasound examination.There were no operative residual or recurrent lesions were found and the scar was little and cryptic.Conclusions The technique of Mammotome minimally invasion surgery in non-palpable breast lesion has some advantages such as direct positioning, easy operation, small pain, fast healing and satisfactory cosmetic effect.It is worthy of clinical application widely.
