摘 要:目的探讨急性脑梗死患者肺部感染中西医结合治疗的疗效,并观察医院感染患者常见病原菌分布,为控制和减少急性脑梗死患者肺部感染的发生提供参考依据。方法选取2013年1月-2015年1月医院收治的60例急性脑梗死肺部感染患者作为研究对象,按照临床试验数字随机的方法将患者分为对照组和观察组,各30例,对照组采用常规的西药抗菌药物进行治疗,观察组在对照组常规治疗的基础上联合中药治疗,观察两组患者治疗后的疗效,数据采用SPSS 17.0软件进行统计分析。结果患者治疗后总有效率观察组为96.7%,高于对照组的83.3%,差异有统计学意义(P<0.05);共检出64株病原菌,以革兰阴性菌为主,共41株占64.1%;观察组患者脑卒中次数、美国国立卫生院神经功能缺损评分均低于对照组(P<0.05)。结论中西医结合治疗急性脑梗死肺部感染效果显著,且改善患者神经功能情况,合理使用抗菌药物,联合中药治疗,可提高疗效。OBJECTIVE To investigate the therapeutic effect of integrative Chinese and western medicine for treatment of pulmonary infections in patients with acute cerebral infarction and to observe the distribution of, common pathogens causing hospital infections so as to provide reference for the control and reduction of pulmonary infections in patients with acute cerebral infarction. METHODS Totally 60 cases of acute cerebral infarction with pulmonary infections in Jan. 2013 to Jan. 2015 were selected as research subjects. According to randomized digital method for grouping in clinical trials, the patients were divided into the control group and the observation groups of 30 patients in each group. The control group received conventional western medicine and the observation group received combined therapy with traditional Chinese medicine based on the conventional western medicine treatment. Efficacy was observed in the two groups after treatment. The software SPSS17.0 was used for statistical processing and analysis of all data. RESULTS The total effective rate was 96.7 % in the observation group, higher than 83.3% in the control group (P〈0.05). Totally 64 pathogens were found, mainly including 41 strains of gram-negative bacteria, accounting for 64.1%. The number of stroke and the neurological deficit scores by the United States National Institutes of Health were significantly lower in the observation group than in the control group (P〈0.05). CONCLUSION The combination therapy for acute cerebral infarction complicated by pulmonary infections has significant efficacy, and can significantly improve neurological function in patients. Clinicians can select the appropriate antimicrobial agents combined with traditional Chinese medicine to improve the efficacy, which is worthy of recommendation.
