子宫动脉结扎加宫腔填塞治疗顽固性产后出血的临床研究

摘　　要：目的对于孕妇顽固性产后出血症状来说,采用子宫动脉结扎加宫腔填塞进行治疗,观察并分析这种治疗方法的临床应用效果及价值。方法选取从2013年3月至2015年3月在我院剖宫产术中治疗顽固性出血的患者40例(均无其它疾病)作为研究对象,随机分成两组,每组20人。分别是试验组和对照组,对照组单独采用单纯的宫腔纱布填塞术,试验组采用子宫动脉结扎+宫腔纱布填塞术治疗,两组患者均是在常规治疗基础之上,一段时间后观察临床情况。结果经治疗后,分析两组患者分别使用不同方式治疗后的止血效果,试验组止血成功率19(93.3%)明显优于对照组16(80.0%),统计学上有意义(P<0.05);患者的术后各项指标均高于对照组。结论在应用常规西医治疗的同时,采用子宫动脉结扎加宫腔填塞进行治疗孕妇顽固性产后出血的临床疗效及安全性比常规采用单纯的宫腔纱布填塞术的临床疗效较显著,可使患者的症状得到明显改善,而且不良反应发生率小,安全可靠,可以在临床上广泛推广使用。Objective： To observe and analyze the clinical effect and value of intractable postpartum hemorrhage treatment by used uterine artery ligation with uterine tamponade. Methods： 40 cases with intractable postpartum hemorrhage,which admitted in our hospital during March 2013 to March 2015,were selected as the research object and divided into the observation group and the control group,each group of 20 cases. Control group was given uterine tamponade treatment and observation group was given uterine artery ligation with uterine tamponade. All of the treatment was based on regular treatment,the clinical situations after a period of time was observed. Results： After treatment different ways of hemostatic effect of two groups were analyzed. The hemostatic success rate of the observation group 19（ 93. 3%） was obviously better than that of the control group 16（ 80. 0%）,he difference was significant statistically significant（ P〈0. 05）. postoperative indicators of the observation group were higher than the control group. Conclusion： In the application of conventional western medicine treatment at the same time,the clinical curative effect and safety of pregnant women with intractable postpartum hemorrhage treated by uterine artery ligation with uterine tamponade is significant batter than conventional uterine tamponade treatment. It can make the patient＇s symptoms improved obviously,and the incidence of adverse reactions,safe and reliable,and can be widely used in clinic.
