摘　　要：目的:探讨不同性别Stanford A型急性主动脉夹层患者的临床特征差异。方法:选取2013年7月至2014年7月,在首都医科大学附属北京安贞医院确诊为A型急性主动脉夹层的患者304例,将患者按性别分为男性组(253例)与女性组(51例)。分析比较两组患者的临床资料,包括:一般情况、既往史、入院实验室及影像学检查、住院期间用药及住院期间不良事件发生情况。结果:两组患者的临床基线资料差别显著,男性患者平均年龄显著小于女性患者,同时患有糖尿病的男性患者显著多于女性患者(P<0.05);两者住院期间用药总体类似,男性患者钙离子拮抗剂的使用率显著高于女性患者,同样,男性患者急性心力衰竭的发生率高于女性患者(P<0.05)。结论:A型急性主动脉夹层男性患者较女性患者常见,且住院不良事件发生率更高,而吸烟史和低密度脂蛋白与住院期间急性心力衰竭发生具有显著相关性,需要临床医生提高警惕。Objective： To analyze the differences of gender-related clinical characteristic of Stanford A type acute aortic dissection patient and in-hospital. Methods： 304 in-hospital patients who came to Beijing Anzhen hospital and diagnosed as Stanford type A acute aortic dissection from July 2013 to July 2014 were included. They were divided into two groups according to gender. Clinical data were analyzed,including： patient characteristics,previous history,results of laboratory and imageological examinations,and in-hospital drug usage and in-hospital incidence rate of adverse events. Results： Significant difference was found in average age between male and female patient（ P 0. 05）. In-hospital usage of calcium ion antagonist was higher in male patient than female patient. Moreover,male patient had significant higher incidence rate of acute heart failure than female patient. Conclusion： Male patients with type A acute aortic dissection patient are more common than female patients,who have higher rate of adverse events in hospital. Meanwhile,smoking history and low density lipoprotein were significantly associated with the occurrence of acute heart failure in hospital. More attention need to paid to those patients.
