摘 要:目的观察急性心肌梗死患者血浆miRNA-133(miR-133)表达情况和体外反搏治疗(ECP)对其水平的影响。方法将60例急性心肌梗死经皮冠状动脉介入(PCI)术后患者随机分为治疗组和对照组各30例,并选择同期门诊健康体检者30例作为空白组。所有受试者均于入院时抽血,采用实时荧光定量PCR(qRT-PCR)法检测血浆miR-133水平。治疗组和对照组给予常规冠心病二级预防药物治疗,治疗组在PCI术后第3周加用ECP,1 h/(次·d),每周5次,为期7周;2组均于治疗9周后分别测定患者血浆miR-133表达水平,评估心绞痛症状改善情况,同时观察超声心动图心功能各项指标变化情况。结果与空白组比较,治疗组急性心肌梗死患者血浆miR-133表达水平明显升高(P<0.05);治疗9周后,治疗组血浆miR-133含量明显低于对照组(P<0.05),心绞痛症状缓解情况及左室射血分数(LVEF)、收缩末体积(ESV)、舒张末体积(EDV)改善情况均明显优于对照组(P均<0.05)。结论血浆miR-133水平可作为急性心肌梗死患者发病和病情进展的一项新型标志物;ECP治疗可降低急性心肌梗死患者血浆miR-133表达水平,明显改善患者心功能。Objective It is to observe the plasma expression level of microRNA- 133( miRNA -133) in patients with acute myocardial infarction (AMI) and to investigate the effects of external counterpulsation(ECP) on this level. Methods 60 AMI patients of after PCI were randomly divided into the treatment group( n = 30) and negative control group( n = 30), and 30 cases outpatients of healthy physical examination as a blank control group (n = 30). The plasma expression level of miRNA - 133 in three groups were analyzed by quantitative Real -Time PCR (qRT - PCR). The treatment group and the negative control group received conventional drug therapy for secondary prevention of coronary heart disease. And then the treatment group launch ECP at 3th weeks after PCI, 1 hour/time o d, 5 times a week for 7 weeks; miRNA - 133 levels ,the improvement of angina symptoms and heart function in two groups of AMI patients were measured by qRT - PCR and echocardiographic indicators respectively after ECP treatment. Results Compared with the blank control group, the plasma expression levels of miRNA - 133 in the AMI patients were significantly elevated (P 〈 0.05 ) ; the level of miRNA - 133 were significantly reduced in the AMI patients after 7 weeks ECP, accompany with clinical symptoms relieved and the echocardiography index of heart function markly improved( P 〈 0. 05 ). Conclusion Circulating miRNA -133 can be used as potential novel biomarker for AMI ; ECP obviously decreased the plasma level of miRNA - 133 in AMI patients and improved cardiac function effectively.
