摘　　要：目的探讨氢质子磁共振波谱(1H magnetic resonance spectroscopy1H M R S)研究儿童急性淋巴性白血病(pediatric acute lymphoblastic leukemia ALL)化疗前后大脑的临床价值。方法对20例儿童(2-10岁)初发ALL患者和20例正常儿童(2-10岁)志愿者进行1H MRS检查,观察患者治疗前、完全缓解后与正常对照组间N-乙酰天门冬氨酸(NAA)、胆碱(Cho)肌酸(Cr)、乳酸(Lac)及NAA/Cr、NAA/Cho、Cho/Cr值的变化,按韦氏儿童智力量表第四版(WISC-IV)对正常儿童及治疗前后ALL患者分别行智力评分,分析这些指标与儿童智商的相关性;结果完全缓解后儿童ALL患者较正常对照组NAA/Cho、NAA/Cr值下降,而Cho/Cr值上升;NAA/Cr、NAA/Cho、Cho/Cr值变化与完全缓解后ALL患者IQ存在相关性;结论 1H MRS对ALL患者完全缓解后智力评估有重要价值。Objective To investigate the value of proton magnetic resonance spectroscopy（1H MRS） in the pediatric patients after chemotherapy with acute lymphoblastic leukemia（ALL）. Methods 20 pediatric patients（2-10 years） with ALL and 20 pediatric volunteers（2-10 years） were examined with 1H MRS, the relationship between MRS values[N-acetylasparate（NAA）, Choline（Cho）, Creatine（Cr）, Lactate（Lac）, NAA/Cr, NAA/Cho, Cho/Cr] and intelligence quotient（IQ） by WISC-IV were analyzed. Results NAA/Cho and NAA/Cr in complete remission（CR） ALL patients were diminished, as compared with normal volunteers, Meantime, MRS values were significantly correlated with IQ in CR patients with ALL. Conclusion MRS has an important value to assess the intelligence of CR patients with ALL.
