摘  要:[目的]探究非正压接头和改良封管手法对降低导管相关血流感染(CRBSI)发生率的作用。[方法]将373例术中通过颈内静脉置入单腔中心静脉导管病人随机分为对照组125例、观察1组124例和观察2组124例,均使用脉冲式正压冲管方式,同时分别对3组病人采用不同接头及封管手法的护理干预,对照组采用传统可来福正压接头,观察1组采用BDQ-Syte非正压接头,观察2组采用BDQ-Syte非正压接头基础上改进封管手法。跟踪4个月后比较3组CRBSI的发生率。[结果]对照组病人CRBSI发生率为2.66/千导管日,观察1组病人CRBSI发生率为1.34/千导管日,观察2组病人CRBSI发生率为0,3组病人CRBSI发生率比较差异有统计学意义(P<0.05)。[结论]使用非正压接头并改良封管手法能降低CRBSI发生风险。Objective:To probe into the effect of non positive pressure joint combined with modified sealing tube method for reducing the risk of catheter related blood stream infection(CRBSI).Methods:A total of 373 patients with single lumen of central venous catheter indwelling into the jugular vein were randomly divided into three groups,125 cases in control group,124 cases in observation groupⅠand 124 cases in observation groupⅡ.The impulse type positive pressure washpipe was used for the patients in three groups.At the same time,three groups of patients were given nursing intervention with different joints and different sealing tube method.The patients in control group used the traditional Kelaifu positive pressure joint,observation group I using Q-Syte BD non positive pressure joint,was used for the patients in observation groups,the improved sealing technique was used for the patients in observation groupⅡ on the basis of Q-Syte BD non positive pressure joint.The incidence of CRBSI was compared among the three groups after follow-up for four months.Results:The incidence of CRBSI was 2.66/1 000 in control group.The incidence of CRBSI was 1.34/1 000 in observation groupⅠ.The incidence of CRBSI was 0/1 000 in observation group Ⅱ.There was statistically significant difference in the incidence of CRBSI among three grollps(P〈0.05).Conclusion:The non positive pressure joint combined with modified sealing tube method could reduce the risk of CRBSI.
