多层螺旋CT在脐尿管病变中的诊断价值

摘　　要：目的 探讨脐尿管病变的多层螺旋CT（MSCT）表现及其临床诊断价值。方法 对我院2009年10月~2014年10月经手术及病理证实的脐尿管病变15例进行回顾性分析,包括病灶部位、大小、形态及Retzius间隙的影像改变。结果 15例患者中,单纯性囊肿2例,MSCT表现为椭圆形、长条状囊性病灶,壁薄;脐尿管囊肿伴感染2例,表现为囊壁增厚,Retzius间隙内可见絮状、条索状密度增高影;脐尿管瘘道并感染3例,其中1例伴脐尿管膀胱憩室;脐尿管瘘并感染2例;脐尿管膀胱憩室6例,伴结石4例。结论 由于其独特的解剖部位及特征性的影像学表现,MSCT可以提高对脐尿管病变的诊断和鉴别诊断能力。Objective To analyze the multislice spiral computer tomography （MSCT） manifestations and its clinical diag- nostic value in urachal abnormalities. Methods A total of 15 patients with urachus disease, who were diagnosed by patho- logical examination in our hospital between October 2009 and October 2014, were included in this study. We retrospective- ly analyzed MSCT images on the location, size, shape and the image features of Retzius space. Results Among them, 2 cases had simple urachal cyst with homogeneous fluid filling the cavity with thin wall in MSCT scanning; 2 cases had infec ted urachal cyst with thickened and enhanced wall, some patch and strip appearing in Retzius space; 3 cases had infected urachal sinus, one case with urachal diverticulum accompanied; 2 cases had urachal leakage. 3 cases had had infected ura- chus leakage; 6 cases had urachal diverticulum accompanied, 4 cases of them with calculus. Conclusion Because of special anatomic positiong and the characteristic imaging features, MSCT scan can provide valuable information for the diagnosis and differential diagnosis of urachal abnormalities.
