消化内镜消毒灭菌效果的影响因素分析与质量管理

摘　　要：目的：探讨消化内镜消毒灭菌效果的影响因素及质量管理。方法：收集2012年10月~2013年12月我院320套使用过后的食管镜、胃镜、十二指肠镜、大肠镜、小肠镜、胆道镜的消毒灭菌效果进行监测,分三个阶段分析消化内镜的消毒灭菌效果。第一阶段为卫生部有关消化内镜清洗消毒灭菌标准执行前期、第二阶段为标准执行初期、第三阶段为标准执行成熟期。按照监测后的结果,探讨影响消化内镜消毒灭菌的因素并实施管理,比较实施管理前后的消毒灭菌效果。结果：标准执行前合格220套,初期合格254套,成熟期合格295套;执行消化内镜清洗消毒灭菌标准前、执行初期、执行成熟期的消化内镜消毒灭菌合格率比较差异有统计学意义（P〈0.05）。结论：消化内镜消毒效果受多种因素影响,消毒质量不达标是医源性传播的重要原因;消化内镜清洗消毒规范执行前及执行初期,消毒灭菌不完善,护士责任心不够,消毒液的监测及管理不当,未注意细节,清洗不彻底等因素是造成消毒灭菌合格率低的主要原因;有效的管理可以减少消化内镜消毒灭菌的不合格率,值得在临床上广泛推广。Objective： Discussion digestive endoscopy sterilization effect of influencing factors and quality management. Methods： October 2012- December2013 in our hospital after used 320 sets of esophagoscopy,gastroscopy sterilization effect,duodenoscopy,colonoscopy,enteroscopy,cholangioscopy be monitored in three stages analysis of digestive endoscopy sterilization effect. The first stage is the ministry of health digestive endoscopy preliminary cleaning and disinfection sterilization standards,and the second stage is the initial implementation of standards,the implementation of the third phase of the standard maturity. In accordance with the results of the monitoring post,to explore the factors affecting digestive endoscopy sterilization and implementation and management,comparative examples before and after sterilization effect management. Results： 220 sets standards qualified,qualified early 254 sets,295 sets of qualified maturity; execution of digestive endoscopy cleaned before sterilization standards,beginning execution,execution mature digestive endoscopy sterilization difference between passing rate statistics Significance（ P 〈 0. 05）. Conclusions： Gastrointestinal Endoscopy disinfection is affected by many factors,disinfection quality standards is an important cause of iatrogenic transmission; digestive endoscopy cleaning and disinfection before the beginning of the implementation and enforcement of norms,inadequate disinfection and sterilization,nurses responsible enough,disinfection Improper monitoring and management solution,no attention to detail,cleaning and other factors is not complete sterilization qualified mainly caused by low; effective management can reduce the failure rate of digestive endoscopy sterilization,it should be popularized in clinic.
