摘 要:目的探讨男性2型糖尿病血清性激素结合球蛋白(SHBG)水平与血脂代谢异常的关系。方法收集男性2型糖尿病患者122例,分别测定血脂谱、性激素结合球蛋白(SHBG)、睾酮、空腹血糖(FBG)、尿酸(UA)、糖化血红蛋白(Hb A1c),分析SHBG与血脂谱的相关性。应用三分位法根据SHBG水平分为低SHBG组、中SHBG组和高SHBG组,采用单因素方差分析研究组间血脂差异。结果相关分析显示,SHBG与甘油三酯(TG)呈负相关(r=-0.39,P<0.05),与高密度脂蛋白胆固醇(HDL)、载脂蛋白A1(apo A1)呈正相关(r分别=0.42、0.37,P均<0.05),校正年龄、病程、体重指数、睾酮因素后,SHBG与HDL、apo A1仍呈正相关(r分别=0.44、0.49,P均<0.05)。单因素方差分析显示,低SHBG组、中SHBG组、高SHBG组的TG水平随着SHBG水平的升高逐渐降低,HDL、apo A1逐渐升高,差异均有统计学意义(F分别=12.59、12.03、15.98,P均<0.05)。结论性激素结合球蛋白可能对男性2型糖尿病血脂代谢有保护作用。Objective To study the relationship between sex hormone-binding globulin (SHBG) and lipid metabolism in men with type 2 diabetes. Methods A total of 122 males with type 2 diabetes were enrolled. Blood fat, sex hormone-binding globulin(SHBG) ,testosterone, fasting blood glucose(FBG),uric acid(UA) and HbA1c were detected and the re-lationship between SHBG and blood fat was analyzed. The patients were divided into low SHBG group, middle SHBG group and high SHBG group according to the level of SHBG. Results SHBG was negatively correlated with TG (r=-0.39,P〈0.05), and positively correlated with HDL-C、apoA1(r=0.42, 0.37,P〈0.05).SHBG was still positively correlated with HDL and apoA1 by controlled age, course, BMI and testosterone (r=0.44, 0.49,P〈0.05). The level of TG was decreased steo by step with the increasing of SHBG, while the HDL and apoA1 were gradually increasing (F=12.59, 12.03,15.98,P〈0.05). Conclusion SHBG might be a protective factor for dyslipidemia in male with type 2 diabetes.
