2型糖尿病患者尿mindin蛋白与颈动脉内膜中层厚度的相关性分析

摘　　要：目的探讨2型糖尿病(T2DM)患者尿mindin蛋白与颈动脉内膜中层厚度(CIMT)的关系。方法选择90例T2DM患者,按尿白蛋白/尿肌酐比值(UACR)分为3组。Ⅰ组(正常蛋白尿组)、Ⅱ组(微量蛋白尿组)、Ⅲ组(大量蛋白尿组),健康体检者32例为对照组。检测各组空腹血糖、餐后2h血糖、糖化血红蛋白(Hb A1c)、血脂、尿mindin/尿肌酐比值(UMCR)、CIMT等。结果 CIMT、UMCR呈对照组、Ⅰ组、Ⅱ组、Ⅲ组递增(P<0.05或0.01)。UMCR与UACR呈正相关(r=0.579、P<0.01);CIMT与UMCR、UACR、病程、Hb A1c、LDL-C、收缩压、血肌酐呈正相关(r=0.385、0.394、0.209、0.199、0.211、0.223、0.202,P<0.05或0.01);CIMT与HDL-C呈负相关(r=-0.218,P<0.05)。多元逐步回归分析显示UMCR、UACR、LDL-C、Hb A1c是对CIMT有独立影响的因素(β=0.332、0.278、0.273、0.267,P<0.05)。结论 UMCR可作为2型糖尿病患者伴发动脉粥样硬化的独立预测指标之一。Objective To investigate the relationship between urinary mindin protein and carotid intima-media thickness（CIMT） in patients with type 2 diabetes mellitus（T2DM）. Methods 90 cases of T2 DM patients,according to urinary albumin/creatinine ratio（UACR） were divided into three groups. Group I（normal albuminuria group）,group Ⅱ（microalbuminuria group） and groupⅢ（macroalbuminuria group）,healthy 32 cases as the control group. We detected the fasting blood glucose,postprandial 2h blood glucose,glycosylated hemoglobin（Hb A1c）,blood lipid,urinary mindin/ creatinine ratio UMCR and carotid artery intima-medial thickness（CIMT）. Results CIMT and UMCR were significantly increased by degrees in the the control group,groupⅠ,group Ⅱand group Ⅲ（P〈0.05 or 0.01）. UMCR and UACR were positively correlated（r=0.579,P〈0.01）;CIMT and UMCR,UACR,disease duration,Hb A1 c,LDL-C had a significant positive correlation（r=0.385,0.394,0.209,0.199,0.211,0.223,0.202,P〈0.05 or 0.01）;CIMT and HDL-C were negative correlated（r=-0.218,P〈0.05）. Multiple stepwise regression analysis showed that UMCR,UACR,LDL-C,Hb A1 C were the independent risk factors of CIMT（β =0.332,0.278,0.273,0.267,P〈0.05）. Conclusion UMCR can be used as an independent predictor of atherosclerosis in patients with type 2 diabetes mellitus.
