摘  要:目的探讨以掌骨内支架系统固定相邻掌骨头下骨折的临床效果。方法自2013年1月至2014年12月,以正常掌骨为内支架通过克氏针对18例相邻的掌骨头下骨折进行固定,并辅助早期的功能锻炼。结果18例22处中,有2处因克氏针针眼感染经换药后控制;其余20处伤口Ⅰ期愈合。术后随访6~24个月,平均12个月。X线片显示骨折愈合时间6~12周,平均8周。按手指总主动活动度评定法评定:优11处,良7处,可3处,差1处;优良率81.8%。结论以掌骨内支架系统固定相邻掌骨头下骨折,手术方法简单,对骨折部位的二次损伤小,疗效满意,是治疗掌骨头下骨折切实可行的术式之一。Objective To investigate the treatment resuhs of subcapital metacarpal fracture fixation with the adjacent metacarpal stent system. Methods From January 2013 to December 2014, 18 patients with subcapital metacarpal fracture were treated by Kirschner wire fixation based on the adjacent normal metacarpal stent system, as well as early functional exercise. Results Among 22 fractures of these 18 patients, needle track infection occurred in 2 cases which was successfully treated by dressing change. The rest all had primary wound healing. Postoperative follow-up ranged from 6 to 24 months with an average of 12 months. X-rays indicated fracture healing was achieved at 6 to 12 weeks with an average of 8 weeks postoperatively. According to the evaluation of the total active motion of the finger, the results were excellent in 11 fractures, good in 7 fractures, fair in 3 fractures and poor in 1 fracture. Conclusion Subcapital metacarpal fracture fixation based on the adjacent metacarpal stentt system is a feasible technique due to the simple application, less traumatic approach, and satisfactory outcomes.
