摘  要:目的:探讨特异性免疫治疗(SIT)对复发性鼻息肉组织中GM-CSF、IL-5表达变化的影响机制及临床意义。方法:采用随机分组方法将确诊为常年性变应性鼻炎伴复发性鼻息肉的患者随机分成2组,实验组19例行SIT+规范化治疗(糖皮质激素喷鼻);对照组17例行规范化治疗(糖皮质激素喷鼻)。分别将2组患者治疗前与治疗半年、1年后鼻息肉组织中GM-CSF、IL-5表达结果进行对比,采用ELISA法检测鼻息肉组织匀浆中GM-CSF、IL-5水平。结果:2组患者治疗半年及1年后鼻息肉组织中GM-CSF、IL-5表达均明显降低;实验组GM-CSF、IL-5表达水平低于对照组,2组差异有统计学意义(P<0.05)。结论:SIT可降低复发性鼻息肉组织中GM-CSF、IL-5细胞因子表达,减轻鼻息肉的炎症反应。Objective:To study the mechanism and clinical significance of specific immunotherapy(SIT)on the expression changes of GM-CSF and IL-5in the tissue samples of recurrent nasal polyps.Method:Perennial allergic rhinitis patients with recurrent nasal polyps were randomly divided into 2groups.The experimental group of 19 patients was treated by SIT and standardized treatment(glucocorticoid nasal spray),and the control group of 17 patients was only treated by standardized treatment(glucocorticoid nasal spray).We measured the expression levels of GM-CSF and IL-5in the tissue samples of the nasal polyps by ELISA,and compared the results obtained before treatment with expression levels detected at 6months and 1year after the treatment.Result:The expression of GM-CSF and IL-5in the recurrent nasal polyps reduced significantly(P〈0.05)in both groups after 6months and1 year post-treatment compared with pre-treatment,and the expression of GM-CSF and IL-5in the experimental group was much lower than that of the control group.Conclusion:SIT decreases the expression of GM-CSF and IL-5and reduces the inflammatory reaction in the tissue samples of recurrent nasal polyps.
