胸腔镜胸腺切除37例分析

摘　　要：目的总结胸腔镜胸腺切除的安全性、适应证及操作要点。方法收集徐州医学院附属医院心胸外科2010年1月至2014年12月经胸腔镜胸腺切除患者37例。其中男性16例，女性21例，平均年龄57．2岁。合并重症肌无力者4例。结果全组手术顺利，无围术期死亡。平均手术时间110（50～210）min，平均出血量70（30～200）ml，术后胸管平均放置时间4．2（2—7）天，术后平均住院时间9．2（6—21）天，医院病死率为0。术后病理证实胸腺瘤23例、胸腺囊肿6例、胸腺增生8例。术后平均随访2．2（0．5～4）年，4例重症肌无力患者3例有不同程度缓解。结论经胸腔镜胸腺切除安全可行，治疗胸腺瘤和重症肌无力的短期随诊疗效满意。Objective To investigate the safety, indication and key surgical procedures of video - assisted thoraco- scopic thymectomy. Methods Clinical data were collected from 37 patients who underwent video - assisted thoracoscop- ic thymectomy from May 2010 to December 2014. Among these patients were 16 men and 21 women, with an average age of 57.2 years old. There were 4 patients with myasthenia gravis. Results All the surgeries were successfully completed without death. The mean operative time was 110 （50 -210） rain. The mean blood loss was 70 （30 -210 ） ml. The mean thoracic drainage time was 4.2 （ 2 - 7 ） days. The mean duration of hospitalization was 9.2 （ 6 - 21 ） days. Ac- cording to pathological results, there were 23 patients with thymomas, 6 ones with thymic cysts, and 8 thymic hyperplasia patients. The mean time of follow - up visits were 2.2 （0.5 - 4） years. Among the four myasthenia gravis patients were three showing remission to various degrees. Conclusions Video - assisted thoracoscopic thymectomy is safe and feasible to treat thymoma and myasthenia gravis, with satisfactory efficacy in a short - term period of follow - up visits.
