摘 要:目的研究不同光疗时问对新生儿高胆红素血症的疗效及不良反应。方法选择120例中等程度新生儿非溶血性高胆红素血症患儿为研究对象,随机分为3组:光疗8h组,光疗12h组,光疗24h组,每组各40例。光疗前、第1次光疗后、治疗后3天分别测血清总胆红素(TSB)及每次光疗后2h测经皮胆红素(TCB),TCB〈205μmol/L停止光疗,同时记录各组黄疸消退时间及不良反应。结果3组第1次光疗后及治疗3天后,黄疸消退程度及黄疸消退时间差异均无统计学意义(P〉0.05),而光疗的不良反应3组之问差异有统计学意义(P〈0.05),随着光疗时间的延长不良反应增加。结论单次光疗8h治疗新生儿高胆红素血症即可达到单次光疗12h和24h的疗效,而且不良反应少。Objective To investigate the effects of different phototherapy durations on neonatal hyperbilirubinemia and adverse reactions. Methods A total of 120 newborn infants with moderate to high non -hemolytic hyperbilirubinemia were enrolled in the study. They were randomly divided into three groups according to the duration of phototherapy given : 8 hours, 12 hours and 24 hours (n =40 for each). The level of total serum bilirubin (TSB) was measured before pbotothera- py, after the first phototherapy and three days after treatment. The amount of transcutaneous bilirubin (TCB) was deter- mined 2 hours after each phototherapy, and phototherapy was continued until TCB 〈 205 p^mol/L. Meanwhile, the duration that neonatal jaundice appeared in each group and adverse reactions were recorded. Results After the first phototherapy and three days after treatment, no significant difference was found as to the rate and duration that neonatal jaundice ap- peared among these groups (P 〉 0.05). However, these groups were remarkably different in adverse reactions which were increased along phototherapy extended (P 〈 0.05 ). Conclusion Single phototherapy for 8 hours can produce the same ef- ficacy after 12 hours and 24 hours of treatment, with less adverse reactions.
