胆道镜联合输尿管镜气压弹道碎石治疗难取性胆管结石的效果分析

摘　　要：目的探讨胆道镜联合输尿管镜气压弹道碎石对难取性胆管结石的治疗价值。方法 选取2014年5月-2015年8月在本院诊治的87例难取性胆管结石患者,将其随机分为观察组（n=44）和对照组（n=43）。观察组采取胆道镜联合输尿管镜气压弹道碎石术治疗,对照组采用传统开腹手术治疗,对比两组的并发症发生率、手术时间、住院时间以及结石清除率。结果 观察组并发症发生率为6.82%,手术时间和住院时间分别为（43.26±4.12）min和（7.11±1.82）d,结石清除率为95.45%;对照组并发症发生率为23.26%,手术时间和住院时间分别为（98.49±16.31）min和（12.08±2.55）d,结石清除率为86.05%;两组上述观察指标差异有统计学意义（P〈0.05）。结论 胆道镜联合输尿管镜气压弹道碎石对难取性胆管结石的治疗效果确切,结石清除率高,并发症较少,手术时间短,且康复较快,值得临床推广应用。Objective To study the treatment value of choledochoscope combined with ureteroscope pneumatic lithotripsy for refractory biliary stones. Methods 87 cases of refractory biliary stones treated in our hospital from May 2014 to August 2015 were selected and randomly divided into observation group（n=44）and control group（n=43）.The observation group was treated with choledochoscope combined with ureteroscope pneumatic lithotripsy,and the control group was treated with traditional laparotomy.The complication incidence rate,operation time,length of stay,stone clearance rate in two groups was compared. Results The complication incidence rate in observation group was 6.82%,the operation time and length of stay was （43.26±4.12） min and（7.11±1.82） d,and the stone clearance rate was 95.45%.In the control group, the complication incidence rate was 23.26%,the operation time and length of stay was （98.49±16.31） min and （12.08± 2.55） d,and the stone clearance rate was 86.05%.The differences were significant in these observation index between two groups（P〈0.05）. Conclusion Choledochoscope combined with ureteroscope pneumatic lithotripsy in the treatment of refractory biliary stone not only has evident effect,but also has the advantages of high stone clearance,fewer complications, short operation time and rapid recovery,it is worth extending in clinical practice.
