高龄前列腺增生患者行经尿道2μm激光气化切除术的疗效及安全性

摘　　要：目的探讨经尿道2μm激光前列腺气化切除术治疗高龄前列腺增生患者的临床疗效及安全性。方法选取2010年7月至2014年7月于北京医院泌尿外科就诊,年龄超过70岁前列腺增生患者213例,采用经尿道2μm激光气化切除术治疗。观察指标包括手术时间、术后保留尿管时间、膀胱冲洗时间。评估基线及术后3个月国际前列腺症状评分、膀胱过度活动症状评分、生活质量评分情况。比较基线、术后24h血红蛋白和电解质的变化及并发症发生情况。结果手术时间32~128min,平均（69±12.4）min;保留尿管时间3~5d,平均（3.6±0.7）d;膀胱冲洗时间18~28h,平均（23±5.1）h。术后3个月各项评分均较基线有明显改善,差异有统计学意义（P〈0.05）。术后血红蛋白和电解质较基线无明显变化,围手术期未出现心、脑及肺等严重并发症。结论高龄前列腺增生患者采用经尿道2μm激光治疗,其围手术期安全性较高,手术时间、保留尿管时间和膀胱冲洗时间缩短,术后恢复快,疗效显著。Objective To evaluate the clinical efficacy and safety of RevoLix2μm continuous wave laser vaporesection in the treatment of benign prostatic hyperplasia（BPH）in senile patients.Methods A total of 213 senile patients with BPH were enrolled,who were more than70 years old.All patients received70 W2μm continuous-wave laser treatment during Jul.2010 and Jul.2014.Resection time,bladder irrigation time and catheter time were recorded.The following variables were assessed before and after surgery,including bleeding volume,electrolyte changes and the complications.International prostatic symptom scores（IPSS）,（overactive bladder symptom score（OABSS）and Quality of Life（QoL）of all patients were recorded and compared before and 3 months after operation.Results The mean resection time was[69±12.4（32~128）]min.The time of bladder irrigation time was[23±5.1（8~28）]h.The mean catheter time was[3.6±0.7（3~5）]d.No severe cardiovascular,cerebral and pulmonary complications occurred during operation.IPSS,OABSS,and QoL were significantly improved 3 months after treatment（P〈0.05）.Conclusions The RevoLix 2μm continuous wave laser vaporesection is safe and effective in the treatment of senile patients with BPH,which has great advantages of clinical efficacy and quick recovery.
