布地奈德混悬液联合阿奇霉素序贯治疗幼儿支原体肺炎

摘　　要：目的观察阿奇霉素序贯治疗联合布地奈德混悬液治疗幼儿支原体肺炎的临床疗效。方法选择2014年1月～12月在我院接受治疗的支原体肺炎患儿148例，按照随机数字表法分为观察组与对照组各74例，对照组在常规治疗的基础上给予乳糖红霉素联合小儿咳喘灵口服液治疗，观察组在常规治疗的基础上给予阿奇霉素（6～10mg／kg·d）及阿奇霉素颗粒（第1d，10mg／kg，2～4d，5mg／kg）序贯治疗联合布地奈德混悬液（1ml／次·d）治疗。1个疗程后观察两组临床疗效及不良反应。结果观察组治疗后咳嗽消失时间、退热时间、肺部哆音消失时间、X线胸片恢复时间、平均住院时间均显著低于对照组（均P〈0．05）；观察组治疗前后IgG、IgM、CD4^＋、CD8^＋及CD4^＋／CD8^＋。差值均显著优于对照组（均P〈0．05）；观察组治疗总有效率显著高于对照组（P〈0．05）；两组不良反应率比较，差异无统计学意义（P〉0．05）。结论阿奇霉素序贯治疗联合普米克令舒在治疗幼儿支原体肺炎方面具有一定的优势，值得临床推广应用。Objective To study the clinical effect of sequential azithromycin and budesonide suspension on treat- ment of children with Mycoplasma pneumonia. Methods 148 patients（age 2.5~ 12） with Mycoplasma pneumonia were divided into group observation and group control according to the method of random digits table. The general data of two groups patients had no statistical difference（P〉0.05）. Group control were given Lactose red enzyme and Children Kech- uan oral liquid treatment on the basis of conventional treatment. Group observation were given sequential azithromycin （64 10 mg/kg· d） and azithromycin granules（ ld 10mg · kg, 2~4d 5mg/kg） and budesonide suspension （1ml/order/d） treatment on the basis of conventional treatment. The clinical effect and adverse reaction were observed. Results After treatment, cough disappearance time, fever clearance time, pulminary rales disappearance time, chest radiography normal time and average hospitalization time in group observation were significantly shorter than that in group control （P 〈0.05）. IgG, IgM, CD4＋ , CD8＋ and CD4＋/CD8＋ of group observation were significantly high than that of group control （P〈0.05）. The total efficiency of group observation was higher than that in group control （P〈0.05）. There were no difference of adverse reaction rate between the two groups（P〈0.05）. Conclusion Sequential azithromycin and budes- onide suspension has certain advantages on treatment of children with Mycoplasma pneumonia.
