探讨经尿道等离子双极电切术治疗前列腺良性增生的临床疗效

摘　　要：目的探讨经尿道等离子双极电切术治疗前列腺良性增生的临床疗效。方法筛选我院收治的前列腺良性增生患者456例,作为研究对象。其中行经尿道前列腺等离子双极电切术221例患者作为A组,行常规经尿道前列腺电切术235例作为B组,比较两组患者手术用时、术中出血量、最大尿流率及术后并发症发生率等指标。结果 A组患者手术用时、术中出血量、术后继发性出血、术后暂时性尿失禁、术后尿道狭窄发生率明显低于对照组,P<0.05,差异具有统计学意义;两组患者术后前列腺症状评分(IPSS)、最大尿流率比较无显著差异,P>0.05,不具有统计学意义。结论经尿道等离子双极电切术与经尿道前列腺电切术治疗前列腺良性增生疗效无明显差异,但经尿道等离子双极电切术术中出血量较低,手术用时短,手术安全性高,可作为前列腺良性增生手术治疗的首选术式。Objective To discuss the clinical curative effect of transurethral plasma bipolar electronical resection on the treatment of benign prostate hyperplasia. Methods 456 cases with benign prostate hyperplasia, which admitted in our hospital, were selected as the research object. 221 cases of patients in the A group was given the transurethral plasma bipolar electronical resection and another 235 cases in the B group was given the conventional transurethral resection of prostate. The index such as the operation time, intraoperative blood loss, maximum flow rate and postoperative complications rate of the two groups were compared. Results The operation time, intraoperative blood loss, postoperative secondary bleeding, postoperative temporary incontinence and the postoperative stricture formation rate of the A group was significantly lower than control group, the difference was significant statistically significant （P〈0.05）. The international prostate symptom score （IPSS） and maximum flow rate of the two groups had no statistically significant difference （P〉0.05）. Conclusion Clinical curative effect of transurethral plasma bipolar electronical resection and transurethral resection of prostate has no significant difference, however for the transurethral plasma bipolar electronieal resection, the intraoperative blood loss is low, operation time is short, surgery higher safety, which can be used as the operative treatment of benign prostate hyperplasia preferred.
