凉血解毒中药对寻常型银屑病Th1/Th2平衡的影响

摘　　要：目的观察消银解毒免煎颗粒对寻常型银屑病血热证患者外周血中T淋巴细胞亚群及辅助性T细胞1(Th1)/辅助性T细胞2(Th2)平衡的影响,探讨消银解毒颗粒治疗寻常型银屑病(进行期及静止期)血热证的免疫学机理。方法将80例寻常型银屑病(进行期及静止期)血热证患者随机分为治疗组及对照组,每组40例。治疗组给予口服消银解毒免煎颗粒治疗;对照组给予口服消银颗粒治疗。2组均治疗8周。记录患者治疗前后"银屑病皮损面积和严重程度指数评分"(PASI)及外周血中CD3^+,CD4^+,CD8^+,CD4^+/CD8^+,Th1/Th2平衡的变化。结果经过8周治疗后2组患者PASI评分均显著降低(P<0.05);2组均可升高CD4^+含量(P<0.05);治疗组可降低CD8^+含量(P<0.05),对照组CD8^+与治疗前无统计学差异(P>0.05);治疗组可降低Th1含量,对照组Th1与治疗前无统计学差异(P>0.05);治疗组、对照组均可升高Th2含量(P<0.05)。结论消银解毒颗粒能够有效地降低寻常型银屑病(进行期及静止期)血热证患者PASI评分并通过改善患者外周血中CD3^+,CD4^+,CD8^+,CD4^+/CD8^+及Th1/Th2的平衡达到治疗的目的。Objective To investigate Xiaoyin Jiedu formula（ XYJDF）,a Chinese medicine compound formulated by TCM method of cooling blood and detoxification,on peripheral T cell subset and the balance of type 1 helper T cells（ Th1） and type 2 helper T cells（ Th2） of psoriasis vulgaris（ PV） patients with syndrome of blood heat,and to try to explore the potential immunological mechanism of XYJDF on treatment of PV at progression stage and stationary stage. Methods 80 patients（ progression stage and stationary stage） were randomly divided into treatment group and control group,each 40 cases. The patients of the treatment group took orally XYJDF and those of the control group took orally Xiao Yin capsule for consecutive eight weeks. Psoriasis Area and Severity Index score（ PASI） and peripheral CD3~＋,CD4~＋,CD8~＋,and the balances of CD4~＋/ CD8~＋and Th1 / Th2 were recorded before and after treatment. Results After eight-week-treatment,PASI scores decreased and the contents of CD4~＋increased both in two groups（ P 0. 05）,but only in the treatment group the content of CD8~＋and Thl decreased（ P 0. 05）. Conclusion XYJDF could effectively reduce PASI score of the psoriasis vulgaris patients with syndrome of blood heat by the way of regulating patient blood contents CD3~＋,CD4~＋and CD8~＋,as well as the balance of CD4~＋/ CD8~＋and Th1 / Th2.
