摘 要:目的探讨外周血淋巴细胞免疫分型及NK细胞水平变化对于间质性肺病变的原因是结缔组织疾病所致,还是其它原因所致的临床价值。方法 50例间质性肺病变患者,分结缔组织疾病组、非结缔组织疾病组,每组25例;正常对照组25例,采用流式细胞术检测外周血中CD3^+、CD4^+、CD8^+、CD56^+NK细胞、CD19^+B淋巴细胞表达水平,比较三组间的外周血淋巴细胞各亚群的差异。结果结缔组织疾病组与非结缔组织疾病组相比,CD8^+T细胞明显增高(P<0.01);结缔组织疾病组与正常对照组相比,CD8^+T细胞也明显增高,但CD56^+数明显降低,均P<0.05;非结缔组织疾病组与正常对照组相比,外周血T细胞各亚群均无明显差异。结论结缔组织疾病组外周血CD8^+T细胞明显增高、NK细胞数明显降低;非结缔组织疾病组外周血T细胞各亚群未见明显异常,提示外周血淋巴细胞免疫分型有助于识别免疫性损伤引起的结缔组织疾病并发间质性肺病变。Objective To explore the clinical value of peripheral blood lymphocyte immunophenotyping and CD56+NK cell levels in interstitial lung disease due to connective tissue diseases or other causes.Methods 50 patients with interstitial lung lesions,divided into connective tissue disease group,non-connective tissue disease group,25 cases in each group.25 cases of healthy as the normal control.The levels ofCD3+,CD4+,CD8+,CD56+NK,CD19+B cell in peripheral blood in the three groups were determined by a flow cytometry;compared differences between three groups of peripheral blood lymphocyte subsets.Results Compared with the non-connective tissue disease group,the levels of CD8+cells in the connective tissue disease group were significantly higher(P〈0.01);compared with the normal control group,the levels of CD8+ cells were also significantly higher,but CD56+NK cells were lowered in the connective tissue disease group(both P〈0.05);compared with the normal control group,the levels of CD3+,CD4+,CD8+,CD56+NK,CD19+B cell in peripheral blood in non-connective tissue disease group had no obvious changes(P〈0.05).Conclusion The levels of CD8+cells in peripheral blood were significantly higher,but CD56+NK cells were lowered in the connective tissue disease group;the levels of CD3+,CD4+,CD8+,CD56+NK,CD19+B cell in peripheral blood in nonconnective tissue disease group had no obvious abnormal.It prompted that peripheral blood lymphocyte immunophenotyping helps to identify immune injury of the connective tissue disease complicated with interstitial lung disease.
