针刺“鬼穴”对甲基苯丙胺依赖者焦虑抑郁情绪疗效观察

摘　　要：目的观察针刺"鬼穴"对甲基苯丙胺(MA)依赖者焦虑抑郁情绪的干预效果。方法将60例男性MA依赖者随机分为对照组、针刺Ⅰ组和针刺Ⅱ组。对照组不予任何干预措施,针刺Ⅰ组取双侧内关、神门、足三里、三阴交、华佗夹脊穴(T5、L2),针刺Ⅱ组取水沟、上星、承浆及双侧大陵、间使、申脉、曲池。针刺组均每星期治疗3次,共12次。分别于治疗前,治疗1、2、3、4星期后观察各组汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)的评分情况。结果治疗1星期后,针刺Ⅱ组HAMA、HAMD评分显著低于对照组(P<0.05),治疗2、3、4星期后,针刺Ⅰ组和针刺Ⅱ组HAMA、HAMD评分均显著低于对照组(P<0.05),治疗2、3星期后,针刺Ⅱ组HAMD评分显著低于针刺Ⅰ组(P<0.05)。但治疗4星期后,针刺Ⅰ组与针刺Ⅱ组HAMD评分比较差异无统计学意义(P>0.05)。结论针刺"鬼穴"和针刺前期研究筛选出的有效体穴均可改善MA依赖者的焦虑抑郁情绪,但可能针刺"鬼穴"在治疗中期能更好地改善MA依赖者的抑郁症状。Objective To observe the effect of acupuncture at the ghost points on anxiety and depression in methamphetamine（MA）-dependent subjects. Method Sixty MA-dependent males were randomized into a control group, acupuncture group Ⅰ and acupuncture group Ⅱ. The control group didn＇t receive any intervention; in acupuncture group Ⅰ, bilateral Neiguan（PC6）, Shenmen（HT7）, Sanyinjiao（SP6）, and Jiaji points（EX-B2） were selected; Shuigou（GV26）, Shangxing（GV23）, Chengjiang（CV24）, and bilateral Daling（PC7）, Jianshi（PC5）, Shenmai（BL62）, and Quchi（LI11） were selected in acupuncture group Ⅱ. For the three groups, treatment was given three times a week, totally for 12 times. The Hamilton Anxiety Scale（HAMA） and Hamilton Depression Scale（HAMD） were observed before treatment, and respectively after 1, 2, 3, and 4-week treatment. Result After the first-week treatment, the HAMA and HAMD scores in acupuncture group Ⅱ were significantly lower than that in the control group（P〈0.05）; after the second, third, and fourth week, the HAMA and HAMD scores in acupuncture group Ⅰ and Ⅱ were significantly lower than that in the control group（P〈0.05）; after the second and third week, the HAMD score in acupuncture group Ⅱ was significantly lower than that in acupuncture group Ⅰ（P〈0.05）. However, after 4-week treatment, there was no significant difference in comparing the HAMD score between the two acupuncture groups（P〉0.05）. Conclusion Acupuncture at the ghost points and at the effective points selected according to previous studies both can improve anxiety and depression state in MA-dependent subjects, but acupuncture at the ghost points can possibly produce a more significant effect in improving depression state in MA-dependent subjects in the midway of the treatment.
