摘 要:目的观察六味地黄丸方对IgA肾病大鼠的治疗作用。方法 SD大鼠40只随机分为正常对照组、模型组、雷公藤多苷片组、六味地黄丸方组,各10只。除正常组外均采用运用牛血清白蛋白+脂多糖+四氯化碳方法建立实验性IgAN模型。造模成功后连续灌胃4周,正常对照组及模型组均采用蒸馏水灌胃;雷公藤多苷片组以6 mg/(kg·d)溶于等体积0.9%氯化钠溶液灌胃;六味地黄汤组以大鼠及人体质量与相应体表面积比值计算给药量。分别在每周末检测尿蛋白定量及尿红细胞计数,第4周末处死各组大鼠,进行肝肾功能检测,IgA免疫荧光检测,肾脏组织石蜡切片进行HE、PAS染色,光镜及电镜下观察病理改变及半定量评估。结果六味地黄丸方组IgA肾病大鼠食欲、精神及毛发情况均较模型组相对较好,但仍较正常对照组差,从六味地黄丸方组尿蛋白及红细胞计数与模型组比较,均较模型组降低(P<0.05),六味地黄丸方组较模型组血肌酐及尿素下降(P>0.05)、血浆白蛋白上升(P<0.05);雷公藤多苷片组及六味地黄丸方组肾小球系膜细胞增生和系膜基质增多情况较模型组减轻(P<0.05);电镜下观察各治疗组系膜增生、系膜基质增多及足突融合较模型组轻;六味地黄汤组大鼠免疫荧光IgA沉积较模型组弱,但较雷公藤多苷片组强(P<0.05)。结论六味地黄丸方可在一定程度上减少IgA肾病大鼠尿蛋白及尿红细胞数,改善其肾功能及升高血浆白蛋白,病理上可减轻IgA在肾脏的沉积和改善肾脏病理病变程度。Objective: To examine the therapeutic effect of Liuwei Dihuang Decoction on IgA Nephropathy Rats. Methods: Forty SD rats were equally randomized into 4 groups: the eonrol group, the model group, tripterygium glycosides group, Liuwei Dihuang decoction group. Except the control group, IgAN rat model was established with bovine serum albumin (BSA), lipopolysaccharide (LPS) and carbon tetrachloride (CC14). After 4 weeks of contin- uous perfusion, the control group and the model group were perfused with distilled water, Tripterygium glycosides tablet group with 6mg/kg/d soluble in equal, volume of saline, Liuwei Dihuang Decoction group by rats and human body weight and body surface area ratio of the corresponding calculation of dosage. Respectively in the weekly proteinuria and urinary red blood cell count were detected, and at the 4th weekend, all the rats were sacrificed for liver and kidney function test. IgA was detected by immunofluorescence, and paraffin sections of renal tissue were HE, Masson and PAS staining,observed under light microscope and semi quantitativeassessment of pathological change. Rerults: In Liuwei Dihuang Decoction group,diet,mental and hair of IgA nephropathy rat were better than those of the model group, and worse than those of the control group. The urine protein and red blood cell count of Liuwei Dihuang Decoction group were lower than those of the model group (P〈 0.05),but more than those of Tripterygium Glycosides tablet group (P〈 0.05 );serum creatinine and urea of Liuwei Dihuang Decoction group decreased more than those of the model group (P〉 0.05) and the plasma albumin increased (P〈 0.05). Mesangial cell proliferation and mesangial matrix increase situation of Tripterygium glycosides tablet group and Liuwei Dihuang Decoction was slighter than those of the model group (P〈 0.05).
