喉罩麻醉在下肢骨折手术中的应用

摘　　要：目的探讨喉罩麻醉在下肢骨折手术中应用的临床效果。方法选取我院2013年3月至2015年3月期间因下肢骨折需手术治疗患者80例,随机分为两组,每组40例,分别在术前予以喉罩麻醉(观察组)和气管插管麻醉(对照组),比较两组麻醉后不同时间段MAP和HR、VAS评分、麻醉后参数以及不良情况。结果观察组麻醉后5 min MAP和HR均低于对照组,但手术进行时和手术结束时MAP和HR均恢复正常水平,差异具有统计学意义(P<0.05);观察组麻醉6 h后VAS评分低于对照组,差异具有统计学意义(P<0.05);观察组置入喉罩操作时间少于对照组插管时间(P<0.05),观察组麻醉开始到手术开始时间、术后拔管时间、口腔黏膜损伤例数、咽喉疼痛例数、术后声嘶例数、拔管即刻降压药使用例数、拔管期呛咳例数均少于对照组,差异均具有统计学意义(P<0.05)。结论喉罩麻醉在下肢骨折手术中应用临床效果好,适合临床广泛推广。Objective To investigate the effect of laryngeal mask anesthesia on operation of lower extremity fractures. Method From March 2015 to March 2013, 80 patients with lower limb fractures receiving treatment in our hospital were selected and randomly divided into 2 groups, 40 cases in each group. The patients were treated with laryngeal mask anesthesia （ observation group） and endo- tracheal intubation anesthesia （control group）. The MAP and HR, VAS score, parameters and adverse conditions were compared be- tween the two groups. Results In the observation group , 5 rain after anesthesia, MAP and HR were lower than those in the control group. However, MAP and HR returned to normal levels during/at the end of the surgery, with statistically significant difference （P 〈 0.05 ） , after 6 hours＇ anesthesia, VAS in the observation group scored lower than that of the control group, the difference being statistically significant （P 〈0. 05）. Laryngeal mask anesthesia took the observation group less time than intubated anesthesia in the control group. The observation group is superior to the control group in such aspects as laryngeal mask insertion time, the intubation time, oral mucosal damage, sore throat, postoperative hoarseness, anti - hypertensive drugs extubation and the extubation cough. The difference was statistically significant （P 〈 0. 05）. Conclusion Laryngeal mask anesthesia in operation of lower limb fractures proves to be clinically effective and worthy of further promotion.
