支气管镜下肺泡灌洗治疗支气管扩张合并感染的疗效研究

摘　　要：目的:分析支气管镜下肺泡灌洗治疗支气管扩张合并感染的疗效以及对患者呼吸功能、炎性因子反应的影响。方法:选择96例接受住院治疗的支气管扩张合并感染患者,将其随机分为观察组与对照组,每组48例。观察组接受支气管镜下肺泡灌洗治疗,对照组接受常规治疗,比较两组患者的呼吸功能、炎性因子反应及基质金属蛋白酶水平等差异。结果:①观察组患者接受治疗后的PaO_2、PaO_2/FiO_2水平高于对照组,PaCO_2水平低于对照组,差异有统计学意义(t=4.763,t=5.673,t=5.237;P<0.05);②观察组患者接受治疗后的hs-CRP、TNF-α、IL-8以及PCT水平均明显低于对照组,差异有统计学意义(t=5.382,t=6.182,t=5.938,t=5.092;P<0.05);③观察组患者接受治疗后的血清MMP-2、MMP-9及TIMP-1水平均明显低于对照组,差异有统计学意义(t=4.982,t=5.736,t=5.837;P<0.05)。结论:支气管镜下肺泡灌洗治疗有助于改善支气管扩张合并感染患者的呼吸功能,缓解全身炎性因子反应,减轻肺部组织的损伤。Objective：To analysis effect of fiberoptic bronchoscopy alveolar lavage on bronchiectasis combined infection patients＇ respiratory function and inflammatory reaction.Methods：Chosen bronchiectasis combined infection patients in our hospital as research object,randomly divided into control group with conventional treatment,observation group with fiberoptic bronchoscopy alveolar lavage,compared with respiratory function,inflammatory reaction and MMP level.Results：1）Observation group patients＇ PaO_2,PaO_2/FiO_2 leves were higher than control group,PaCO_2 level was lower than control group（t=4.763,t=5.673,t=5.237;P〈0.05）;2）Observation group patients＇ hs-CRP,TNF-α,IL-8,PCT levels were significantly lower than control group（t=5.382,t=6.182,t=5.938,t=5.092;P〈0.05）;3）Observation group patients＇ serum levels of MMP-2,MMP-9 and TIMP-1 levels were significantly lower than control group（t=4.982,t=5.736,t=5.837;P〈0.05）.Conclusion：Fiberoptic bronchoscopy alveolar lavage contributed to improve respiratory function of patients with bronchiectasis and infection,relieve systemic inflammatory response and lung tissue damage.
