显微支撑喉镜与纤维喉镜下YAG激光手术治疗声带息肉的疗效研究

摘　　要：目的:研究显微支撑喉镜手术与纤维喉镜下YAG激光手术治疗声带息肉的疗效及对术后噪音学的影响。方法:选取130例声带息肉患者,采用随机数表法将其分为观察组与对照组,每组60例,观察组患者接受显微支撑喉镜手术治疗,对照组患者接受纤维喉镜下YAG激光手术。比较两组患者的手术情况、嗓音学指标以及生活质量。结果:1手术情况,观察组患者的整体疗效优于对照组,一次切除成功率高于对照组,复发率低于对照组;2嗓音学指标,观察组患者的基频微扰(Jitter)值、振幅微扰(Shimmer)值低于对照组,声门标准化噪声能量(NNE)的绝对值高于对照组;3生活质量:观察组患者的物质生活、躯体健康、心理健康以及社会功能评分高于对照组。结论:显微支撑喉镜手术的整体效果优于纤维喉镜手术,有助于提高切除成功率、降低复发率及改善嗓音功能,是治疗声带息肉的理想方法。Objective： To study the curative effect and phonetics of micro laryngoscope operation and fiber laryngoscope operation in treating vocal polyps. Methods： One hundred and thirty cases of vocal cord polyp patients in our hospital from 2011 March to 2014 May were enrolled and divided into two groups according to random number table method. Observation group patients received micro laryngoscope operation and control group received YAG laser operation under fibre laryngoscope. Operation situation, phonetics index and quality of life were compared. Results： 1）Operation situation： overall curative effect of observation group was better than that ofn control group, successful rate was higher than control group, recurrence rate was lower than control group; 2）Phonetics indexes： Jitter, Shimmer of observation group were lower than control group, absolute value of NNE was higher than control group; 3）Quality of life： material life, physical health, mental health, social function scores of observation group were higher than those of control group. Conclusion： Overall effect of micro laryngoscope operation is better than fiber laryngoscope operation, improve success rate, reduce recurrence rate and improve voice function; it＇s an ideal method in treating vocal cord polyp.
