摘　　要：目的探讨血液灌流联合血液透析对尿毒症患者血液甲状旁腺激素水平及皮肤瘙痒症状的改善效果。方法选取2012年5月至2015年5月陕西省宝鸡市中心医院肾病内科糖尿病科透析室收治的80例尿毒症及甲状旁腺机能亢进患者,依据数字表法随机分为2组,即研究组(n=40)和对照组(n=40)。对照组患者予以单纯血液透析,研究组予以患者血液灌流联合血液透析治疗,然后对2组患者的甲状旁腺激素(PTH)水平及皮肤瘙痒程度评分进行统计分析。结果研究组患者的PTH水平[(18.3±6.7)μg/L]和皮肤瘙痒程度评分(5.5±1.6)分均显著低于对照组[(23.8±8.3)μg/L,(8.5±1.4)分],差异均有统计学意义(P<0.05),治疗的总有效率[95.0%(38/40)]显著高于对照组[62.5%(25/40)],差异有统计学意义(P<0.05)。结论血液灌流联合血液透析能够有效降低尿毒症患者血液甲状旁腺激素水平,改善患者皮肤瘙痒症状。Objective To investigate the therapeutic effects of hemoperfusion combined with hemodialysis on the serum level of parathyroid hormone and pruritus of the patients with uremia. Methods Eighty cases of uremia combined with hyperparathyroidism admitted into the diabetic dialysis ward of the Nephrology Division,Baoji Central Hospital from May 2012 to May 2015 were recruited as our research subjects. The patients were randomly divided into 2 groups,the study group（ n = 40） and the control group（ n = 40）.The patients in the control group were given hemodialysis alone,while the patients in the study group received hemoperfusion as well as hemodialysis. Then,statistical analyses were made on the parathyroid hormone（ PTH） levels and pruritus severity scores in the patients of the 2 groups. Results PTH level [（ 18. 3 ± 6. 7） μg / L]and pruritus severity score（ 5. 5 ± 1. 6） of the study group were all significantly lower than those of the control group [（ 23. 8 ± 8. 3） μg / L,8. 5 ± 1. 4],with statistical significance（ P〈0. 05）. The total curative efficacy was 95. 0%（ 38 /40）,which was significantly higher than that of the control group [62. 5%（ 25 /40） ]（ P〈0. 05）.Conclusion Hemoperfusion combined with hemodialysis could effectively decrease the serum parathyroid hormone level and improve pruritus of uremic patients.
