糖化白蛋白及糖化血红蛋白在妊娠期糖尿病筛查中的价值

摘　　要：目的:探讨糖化白蛋白(GA)和糖化血红蛋白(HbA1c)检测在妊娠期糖尿病(GDM)患者筛查中的价值。方法:根据国际妊娠与糖尿病研究组织(IADPSG)新诊断标准下对符合标准的产前检查孕妇,以进行75g口服葡萄糖耐量试验(OGTT),结果作为诊断GDM的标准,根据结果将其中100例孕妇分为健康孕妇组50例和GDM组50例,同时测定两组的GA和HbA1c,对其结果进行分析。结果:GDM组与健康孕妇组比较OGTT、GA和HbA1c值都显著升高,差异有统计学意义(P<0.05)。GDM组GA、HbA1c阳性率分别为38%、24%,健康孕妇组GA、HbA1c阳性率分别为10%、4%。两组GA阳性率高于HbA1c。结论:GA和HbA1c可以与OGTT联合使用,提高GDM检出率,降低妊娠期并发症的风险,保证母婴健康安全。Objective：To investigate the value of glycosylated albumin（GA）and glycosylated hemoglobin（HbA1c）in the screening of patients with gestational diabetes mellitus（GDM）.Methods：According to the new diagnostic criteria of the international pregnancy and diabetes research organization,the pregnant women who accord with the standard prenatal examination,75 g oral glucose tolerance test（OGTT）,the results as the diagnostic criteria of GDM.According to the results,100 cases of pregnant women were divided into 50 cases of healthy pregnant women and 50 cases of gestational diabetes,the HbA1 cand GA were determined by two groups,and the results of these two groups were analyzed.Results：Compared with the healthy pregnant women,HbA1 c,GA and OGTT were significantly higher in the GDM group.The difference was statistically significant（P〈0.05）.The positive rates of HbA1 cand GA in GDM group were38% and 24%,and the positive rate of GA and HbA1 cin healthy pregnant women were 10% and 4%.The positive rate of GA in the two groups was higher than that of HbA1 c.Conclusion：GA and HbA1 ccan be used in combination with OGTT to improve the detection rate of GDM and reduce the risk of complications during pregnancy.To ensure the health and safety of the mother and baby.
