摘 要:目的 系统评价能谱CT显示肝癌的最佳单能量。方法 检索PubMed、Web of Knowledge、万方和CNKI等数据库,检索时间截止2014-10。由2名评价者独立筛选文献、提取资料并进行方法学质量评价后,采用RevMan 5.3软件进行Meta分析。结果 共纳入6个研究,188例研究对象。Meta分析结果显示:动脉期,50 keV单能量时显示病灶的对比噪声比(CNR)优于其他单能量水平,P<0.000 01;门脉期,70 keV时较其他单能量显示病灶的CNR最佳,且P<0.000 01。结论 动脉期50 keV、门脉期70 keV是能谱CT显示肝癌的最佳单能量。Objective To systematically evaluate the optimal monochromatic images for detection of liver cancer with spectral CT. Methods Some databases including PubMed, Web of Knowledge, Wanfang Database and China National Knowledge Infrastruc ture (CNKI) before October 2014 were searched. According to strict inclusion and exclusion criteria, two reviewers independently performed literature screening, data extraction and quality evaluation. Meta-analysis was conducted with a Revman 5.3 Software. Results A total of 6 studies involved 188 patients were enrolled in the study. Meta-analysis results showed that the contrast-to-noise ratio (CNR) at 50 keV was better than others in arterial phase (P〈0. 000 01), and the optimal CNR in portal venous phase was at 70 keV (P〈0. 000 01). Conclusion The monochromatic images with 50 keV in arterial phase and 70 keV in portal venous phase are optimal for detection of liver cancer with spectral CT.
