摘  要:目的:探讨超声引导下金标植入在射波刀治疗肝脏肿瘤中的技术要点、效果及并发症。方法:肝脏肿瘤患者33例,其中原发性肝癌21例,转移性12例,共59个瘤体;在超声引导下植入金标,每瘤体置入金标4~6颗,行射波刀放射治疗,并评价治疗效果。结果:33例肝脏肿瘤患者59个瘤体中,最大者9.6cm×8.7cm,最小者为3.1cm×1.7cm;共植入255颗金标,位置较为理想,在治疗中能够使用的金标有251颗,金标的利用率为98.4%。结论:超声引导下置入金标治疗肝脏肿瘤方便、定位准确,并发症少,可避开重要部位及血管,穿刺出血时可监测,有肺部转移时可在近膈肌处放置,同时可照射肺部病变,且经济方便。Objective:To investigate the keypoints,effects and complications of fiducial implantation guided by ultrasonography in the CyberKnife treatment of liver tumors.Methods:The fiducial markers were implanted under ultrasonic guidance for 59 hepatic tumors in 33 patients including 21 cases with primary liver tumors and 12 with liver metastases,and every tumor received 4to 6implanted fiducials.The treatment effectiveness was evaluated for every patient after CyberKnife radiation treatment.Results:A total of 255 fiducials were implanted in 59 tumors with the maximum diameter from 3.1to 9.6cm,and 251 fiducials could be effectively used for CyberKnife treatment,with an effective rate of 98.4%.Conclusion:The ultrasound-guide fiducial marker implantation for liver tumors is a convenient and accurate technique for CyberKnife treatment with less complication.It has advantages of avoiding important organs and blood vessels,real-time monitoring of bleeding and low costs.The fiducial implantation could be performed nearby the diaphragm for CyberKnife treatment of lung metastases.
