摘  要:目的观察胆道疾病伴窦性心动过缓阿托品试验假阳性的发生率,并探索胆道疾病合并病态窦房结综合征(SSS)的有效鉴别手段。方法对本院116例胆道疾病伴窦性心动过缓患者行阿托品试验,阳性患者进一步行动态心电图检查,分析检查结果。结果阿托品试验结果示,86例阴性,30例阳性,阳性率为25.9%;30例阳性患者动态心电图结果示,11例排除SSS,19例确诊为SSS,阿托品试验假阳性率为36.7%。结论在阿托品试验基础上行动态心电图序贯检查,能排除阿托品试验的假阳性。心电图-阿托品试验-动态心电图监测序贯检查,可为临床准确诊断胆道疾病患者是否合并SSS提供更有力的电生理依据,指导诊疗方案的选择。Objective To observe the false positive rate of atropine test in patients of biliary tract diseases associated with sinus bradycardia, and to explore the effective identification method to diagnose the sick sinus syndrome (SSS) in patients of biliary tract diseases. Methods 116 patients with biliary tract diseases associated with sinus bradycardia received the atropine test. The patients with positive resuhs further received dynamic electrocardiogram. The examination results were analyzed. Results The atropine test showed that there were 30 positive and 86 negative cases. The positive rate was 25.9%. The dynamic electrocardiogram of the 30 positive cases demonstrated that 11 cases were not SSS, and 19 cases were SSS. The false positive rate of atropine test was 36.7%. The dynamic electrocardiogram could confirm the sick sinus syndrome in positive patients of atropine tests. Conclusion Atropine test plus dynamic electrocardiogram can eliminate the false positive result of the test. Electrocardiogram-atropine test-dynamic electrocardiogram can provide electroohvsiologic evidence for the diagnosis of SSS and guidance for the selection of treatment oians.
