远端胃间置术治疗不可切除性远端胃癌的疗效观察

摘　　要：目的：探讨远端胃间置术治疗不可切除性远端胃癌的临床疗效.方法：对6例远端晚期胃癌并幽门梗阻、原发肿瘤已不能切除的患者进行远端胃间置术联合近端胃空肠BillrothⅡ吻合（3例）和Roux-en-Y吻合（3例）.结果：所有患者在术后3～5d肛门排气,平均（3.8±1.0）d;3～10 d恢复进食,平均（5.5±2.6）d;术后2个月较术前血清白蛋白增加3.9 ～ 11.4 g/L,平均（6.5±2.7）g/L;体重增加0.8～ 3.3 kg,平均（2.3±1.0）kg;术后生存3～ 22个月,平均（11.8±8.1）个月.结论：远端胃间置术可明显改善不可切除性远端胃癌患者的全身情况、提高生活质量、延长生存时间,是治疗不可切除性远端胃癌的一种有效方法,值得临床推广应用.Objective： To investigate the clinical efficacy of distal stomachpartitioning surgery in treatment of unresectable distal gastric cancer, Methods： 6 cases of unresectable distal gastric cancer with pyloric obstruction were performed the surgery of stomach-partition,in which 3 cases plus proximal stomach jejunum Billroth II anastomosis and the other 3 cases plus stomach jejunum Roux-en-Y anastomosis. Results： Postoperatively, all the patients had anal exhaust at the day 3.8 ± 1.0 （3-5 days）, and restore the diet at the day 5.5 ± 2.6 （3-10 days）, and serum albumin increased （6.5±2.7）g/L （3.9-11.4 g/L） and weight increased （2.3 ± 1.0） kg （0.8-3.3 kg） compare to preoperative two months late. The average survival of all the patients was （11.8 ±8.1 ） months（3-22 months）. Conclusion： The distal stomach- partitioning surgery can obviously improve the general condition, the quality of life and prolong the survival time of unresectable distal gastric cancer patients, which is effective method for the treatment of unresectable distal gastric cancer and worthy of clinical application.
